You spend enough time understanding what went wrong and even identify who was responsible for it and now you're in front of the senior leadership. But when they ask who's fault it was, you take the blame on yourself. The senior leadership is surprised how it could be you since you just returned from your vacation to which you say, it's your team and their actions are your accountability and you're ready to face any consequences that come off it.
