    Dr. Ray Ebarb is a Brooklyn native and a graduate of the State University of New York in Buffalo, where he received his Bachelor of Science in Biochemistry. Then, equipped with only his high school Spanish, he went on to attend medical school in Guadalajara, Jalisco, Mexico for four years. He then came back to Bay Shore, Long Island for his Family Medicine residency. Dr. Ebarb settled down in Sayville, New York in 1991 and started the Great South Bay Medical Practice, where he has continued to practice family medicine for 38 years, and he is particularly experienced in providing geriatric care. Dr. Ebarb has been a member of the NYSAFP since 1984 and was the NYSAFP Past President from 2013-2014. He now serves as the state delegate to the AAFP Congress of Delegates.

    Dr. Jane Simpson attended Edward Via Virginia College of Osteopathic Medicine at Virginia Tech (VCOM) for her medical education and spent her 3rd and 4th years working with underserved populations in West Virginia. Dr Simpson completed her Family Medicine Residency at UHS Wilson Hospital in Binghamton, NY where she served as chief resident and then completed a fellowship in Hospice and Palliative Medicine at Brown University. Dual board certified in Family Medicine and Hospice and Palliative Medicine, Dr Simpson serves as Director of Palliative Medicine at Samaritan Medical Center in Watertown, NY. Dr Simpson serves as the board chair of the NYSAFP, and Vice Speaker of MSSNY. Our topic for today is health disparities in end of life care.

    NYSAFP DEI PODCAST - Medical Aid in Dying Part 1 - Dr. Bruce Ellswig

    Dr. Ellsweig is a board-certified family physician with former certification in hospice and palliative care and currently serves on the Medical Aid in Dying (MAID) sub-commission of the NYSAFP’s Public Health Commission. In 2023, Dr. Ellsweig was selected as Physician of the Year for the New York State Rural Health Association. He graduated from Jefferson Medical College in 1974 and completed his residency at York Hospital in York Pennsylvania in 1977. He practiced family medicine in Allentown Pennsylvania, first in a small group practice and subsequently as part of a not-for-profit hospital network until 2013. During his tenure at the hospital network, he served in multiple leadership roles, including hospice medical director, vice chairman of the Department of Family Medicine, and chairman of a 400+ member Independent Physician Association (IPA) all while actively engaged in various community activities. Dr. Ellsweig then relocated to Sullivan County where he served an underserved population for 2.5 years as the director of primary care and community medicine for a local hospital network.

    Dr. Naz Khan is the Chief Medical Officer for Catholic Health Physician Partners on Long Island, New York and the founding member of the Racial and Ethnic Health Diversity Initiative (REHDI) at Anthem Blue Cross Blue Shield. Dr. Khan has played a pivotal role in various initiatives aimed at addressing disparities in healthcare. She is a key member of the Health Equity Leadership Council at Catholic Health, where she continues to champion policies and programs that promote equitable healthcare access. Dr. Khan leads PEER RISE for Catholic Health Physician Partners, a program designed to empower physicians to support each other, fostering a collaborative and inclusive professional environment. Her remarkable contributions during the COVID-19 pandemic earned her a commendation from the New York Senate for her work in health equity.

    Marc D. Price, D.O. is a board certified Family Physician and is the founding and managing partner of an award-winning progressive small group practice, Family Medicine of Malta, in Malta, N.Y. His practice has, and continues to participate with many Medicare, NCQA (National Committee for Quality Assurance) and health insurance initiatives as well as an accountable care organization (ACO) to improve the quality of healthcare for his patients and his community. He has been active with the NYSAFP (NY State Academy of Family Physicians) and AAFP (American Academy of Family Physicians) since 2000 and is a past President of the NYSAFP and the current Delegate to the AAFP after having served various other state academy roles such as Vice President, Treasurer, Alternate Delegate to the AAFP and Advocacy and Operations Commissions Chair. He has served as the chair of the AAFP Commission for Government Advocacy and the AAFP Commission on Finance and Insurance and has been a member on the AAFP Commission on Quality and Practice. He serves as a medical director to a vaccine distribution company and has had similar roles for a home health care organization and as a local medical director to his ACO. Dr Price is also on the board of directors of MVMA (Mohawk Valley Medical Associates) and is an Assistant Clinical Professor through Albany Medical College and enjoys promoting the practice of family medicine to students of all levels, residents, other healthcare professionals and his community. In his free time he enjoys spending time with family and friends, traveling, riding his motorcycle, cooking and slow and low barbecue.

    Scott Hartman (pronouns he, him) is an Associate Professor of Family Medicine at the University of Rochester Medical Center. He serves on the Board of the NYS Academy of Family Physicians and was previously chair of the AAFP’s Commission on Health of the Public and Science. In addition to his clinical experience and expertise in LGBTQ+ health and HIV medicine, he has been extensively involved in state and national policy and advocacy work,  including policies on gender affirming care, and the passage of the gender affirming care Shield Law in NY State.
