Welcome to TarHeal Wellness, the resident wellness podcast! From the University of North Carolina Hospitals Office of Graduate Medical Education, hosts (Robert Rowe, MD, MBA, MPH, Rui Ariyapala, MD, EdM, Sara Helvey, MD, MPH, MBA, and Daniel Carnegie, MD, MBA, MPH) explore the world of medical residency through interviews with current and former residents and experts in various wellness topics, including self-care, career decisions, professional and social relationships, and much more.     In this two-part episode, Dr. Rui Ariyapala sits down with Dr. Yolanda Gomez, a Board-Certified Pediatrician with an impressive career in both clinical practice and medical education leadership. Dr. Gomez earned her medical degree from the University of Puerto Rico School of Medicine in San Juan before launching her career as a general pediatrician in private practice. After taking a year off to focus on family, she returned to academia, serving as Director of the Primary Care Clinic at the UPR Pediatrics Department, where she supervised students and residents. Her leadership skills quickly propelled her to become the Pediatrics Residency Program Director, a role she held from 1996 to 2001. In 2001, Dr. Gomez was appointed Associate Dean for Graduate Medical Education (GME) and Designated Institutional Official (DIO) at the UPR School of Medicine, stepping into the role just as the institution’s GME office faced probation by the ACGME. Through her efforts, the office was removed from probation in 2003, earning her praise for her transformative changes. She continued to lead in this capacity until 2014, when she joined the University of Texas Health Science Center in San Antonio, helping to establish 11 new residency programs as Associate Dean for GME and DIO at the UT Rio Grande Valley. In 2019, Dr. Gomez transitioned to the University of Oklahoma’s School of Community Medicine in Tulsa, where she currently serves as Associate Dean for GME and DIO. In this role, she oversees ACGME-accredited programs while championing resident wellbeing throughout their training. In part 1 of our conversation, Dr. Gomez shares her personal experiences as a resident, including a pivotal moment when she stood up to an attending physician—a defining moment in her career. In part 2, we dive into her leadership journey and how she continues to prioritize resident wellbeing in her work.

    In this episode, Dr. Rui Ariyapala speaks with Dr. Karen Studer, Program Director of the Public Health and General Preventive Medicine residency at Loma Linda University Health and Assistant Professor at the School of Public Health. Dr. Studer is a clinician at an FQHC, teaching residents and medical students at her women’s lifestyle medicine clinic. She holds an MBA in healthcare administration, board certifications in Lifestyle, Obesity, and Integrative Medicine, and is a founding faculty member of the Lifestyle Medicine Residency Curriculum (LMRC). Since August 2022, she has served as a Director on the American Board of Preventive Medicine. In July 2024, she became Chair of the Department of Preventive Medicine at Loma Linda University Health. Our discussion focuses on medical career paths, applying a growth mindset, wellness, and managing burnout and work-life balance. Dr. Studer shares her unique perspectives and pragmatic tools on improving health not only for her patients but herself.

    In this episode, Dr. Rui Ariyapala speaks with Dr. Andrea Otero Rios. Originally from San Juan, Puerto Rico, Dr. Otero Rios is a first-generation doctor and has been part of several events in Puerto Rico, including Hurricane Maria, the 2021 earthquake, and COVID-19 relief efforts with the Puerto Rico National Guard.  After finishing medical school Dr. Otero Rios became a Neurology Pre-Resident Intern at the University of New Mexico in Albuquerque. Her interests include sports medicine, brain injury, pain management, lifestyle medicine, medical Spanish, and research. She is now in her second year as a resident in family medicine at Texas Tech University Health Sciences Center - El Paso. We discussed her unique journey, where she overcame many challenges in the residency match process. She recommends every listener to read the book: “Attending: Medicine, Mindfulness and Humanity” by Ronald Epstein. Dr. Otero Rios provides useful wellness tips for our listeners on how to have a positive mindset in the face of setbacks and adversity in medical training.

    Episode 66 - Dr. Christina Miller: Staying in the ‘Green Zone’ and How to Stay Resilient

    In this episode Dr Rui Ariyapala speaks with Dr. Christina Metzler Miller. Dr Miller is a graduate of the David Geffen School of Medicine at UCLA and obtained her MPH in Population Medicine at Loma Linda University, where she also completed a Family Medicine and Preventive Medicine Residency. While she was a resident, she founded and led a nonprofit free clinic in the city of San Bernardino.  She completed a fellowship in global health with World Medical Mission in Malawi. She recently co-founded another nonprofit, Cornerstone Group Consultants, assisting a number of organizations with grant writing, research, and publications. Dr. Miller is certified through the American Academy of Family Physicians Leading Physician Wellbeing program and as a lifestyle medicine intensivist with the American Board of Lifestyle Medicine. We delve into how she overcame her most challenging moments of residency and global health fellowship. Dr. Miller discusses how to stay in the “green zone” for wellness, sharing her Plan of Action for Resilience, which became one of her cornerstone practices of finding regenerative joy.

    Episode 65 - Dr. Michael Baca-Atlas: Moral Injury and Wellness Toolkits in Residency

    In this episode, Dr Rui Ariyapala speaks with Dr. Michael Baca-Atlas. Dr. Baca-Atlas is originally from Baltimore, MD. He completed his undergraduate work at the University of Maryland, College Park and medical school at University of Maryland in Baltimore. He completed his Family Medicine residency at UNC and an Addiction Medicine fellowship at UNC in the Department of Psychiatry. He serves as the Medical Director for UNC REACH Enhanced Primary Care and Associate Medical Director for the Tobacco Treatment Program. Dr. Baca-Atlas has a special interest in the intersection of physical and mental health and addiction.  He also serves as a contributor for Primary Care RAP (Reviews and Perspectives), a medical education podcast. We dive into how he switched his career aspirations from OBGYN to Family Medicine and he gives important advice on how to choose your specialty wisely. We talk about the need to triage medical tasks as a resident, to feel empowered and work to the best of your ability. Finally, we discuss how to navigate the interesting concept of moral injury in medicine and address burnout, as he provides insights in building your own ‘wellness toolkit’ during residency and beyond.
