Why 71% of “Hip Pain” Is Actually Coming from Your Back! | Episode 277

Is your hip pain really coming from your hip — or is your back the hidden cause? In this podcast, Stefani Crowley, MSPT, Dip. MDT (McKenzie Institute Diplomate — a credential held by fewer than 1% of physical therapists), breaks down why so many people are treated for hip pain for months or years without ever getting better — because no one checked whether the real source was their spine. You'll learn: ✅ The research showing up to 71% of hip pain can actually originate from the spine ✅ A real case study: a patient with a stiff, painful hip for over a year — steroid injections and PRP didn't help — until we looked at her back instead ✅ Why MRIs and imaging can send you down the wrong path entirely ✅ The specific questions we ask to tell the difference between a true hip problem and a back problem showing up in the hip ✅ Simple ways you can start to figure out, on your own, which one you're dealing with If you've been told your hip is "just wearing out," or you've tried injections, stretching, or rest and nothing has worked — this podcast may explain why. Ready to find your actual source of pain? Join us for our free Back Pain Masterclass on Sept 16th at 6pm at mPower Physical Therapy — Is It Your Hip or Your Back? Why Does My Back Hurt When I Walk — And Why Does Sitting Make It Better? | 276

If your back pain comes on after walking a few minutes — and immediately improves when you sit down — you may have been told it's spinal stenosis. You may have been told there's not much to do. In 25 years of clinical practice, Dr. Stefani Crowley has seen exactly one true case of spinal stenosis that couldn't be significantly improved with the right treatment. Every other patient with this exact presentation improved — because the real problem was something different entirely. In this podcast, she explains what's actually going on, how to tell the difference, and what can change. What you'll learn: ✅ Why walking normally helps back pain — and what changes that ✅ What true spinal stenosis is, and what it actually looks like clinically ✅ The far more common presentation that gets misdiagnosed as stenosis — and how to spot it ✅ Why people with chronic back stiffness lose their extension range — and what that does to walking ✅ The simple self-test you can do right now to begin telling the two apart ✅ The research on asymptomatic stenosis: relative stenosis found in 20% under 40, nearly 50% by age 60-69 (McKenzie Institute Part C) ✅ A patient story: half-block walking tolerance → evening walks restored in 6 visits Based on the McKenzie Institute of Mechanical Diagnosis and Therapy (MDT) clinical framework and published research on lumbar spinal stenosis. You wake up stiff, achy, and shuffling like you've aged overnight — but by 10am you feel almost normal. Sound familiar? In this video, Dr. Stefani Crowley (MSPT, Dip. MDT) explains the science behind morning back pain, why your spine is actually at its most reactive state right when you wake up, and what your pain pattern is telling you about the source of the problem. When the nurse asked how his pain was — he said zero. He got up and walked out. Six visits later at our clinic, he was pain free and never looked back. In this podcast, Dr. Stefani Crowley (MSPT, Dip. MDT) breaks down one of the most important questions people with chronic back pain face: Do I actually need surgery — or has no one properly assessed what's really going on? What you'll learn: ✅ What the Lancet and major clinical guidelines actually say about back surgery as first-line care ✅ Why back pain keeps coming back — even after treatment ✅ The difference between managing pain and fixing the mechanical source ✅ What a "directional preference" is — and why finding it can change everything ✅ Why the McKenzie Method gets results when standard PT hasn't lasted ✅ The one question to ask yourself about your pain right now This podcast is based on principles from the McKenzie Institute of Mechanical Diagnosis and Therapy (MDT) and supported by published research from the Lancet, Spine Journal, and the McKenzie Institute International. Sign up here to save your spot: https://info.mpower-pt.com/Back-pain-masterclass Back pain affects nearly 1 in 3 adults — yet most people are being evaluated with a 10-minute appointment and an MRI report. In this video, I'm going to show you why that approach is failing you, and what a proper whole-body evaluation actually looks like. In this podcast you'll learn: ✅ Why your MRI findings — disc degeneration, facet arthritis, bone spurs, narrowed joint spaces — can be completely NORMAL findings on a pain-free person's scan ✅ The many different structures that can contribute to back pain: your lumbar spine, your lower thoracic spine, your hip, your sacroiliac joint, AND deep muscles like the psoas and iliacus ✅ Why core strengthening, massage, and chiropractic adjustments often fail to give lasting relief ✅ What a proper baseline evaluation looks like — and the process I've refined over 25 years and thousands of patients ✅ Why "mobility before stability" is the key principle most PTs skip ✅ What the McKenzie Method (MDT) is and why it's clinically proven to reduce surgery rates Stefani Crowley, MSPT, Dip. MDT is the owner and lead clinician at mPower Physical Therapy in Dallas, TX. What Is a Knee Derangement? (And Why Your MRI Keeps Missing the Real Problem)| Episode 271

You've been told it's arthritis. Or a meniscal tear. Or just wear and tear. You've done the physical therapy, maybe had an injection, possibly even had surgery, and the pain keeps coming back. There is a very real possibility that a specific, classifiable, treatable mechanical problem has been missed in your knee every single time. It's called a Knee Derangement. And nearly half of all knee pain patients have one. In this podcast, Stefani Crowley, physical therapist and McKenzie Institute Diplomate at mPower Physical Therapy in Dallas, TX, explains exactly what a Knee Derangement is, how it presents, why it is so frequently mistaken for arthritis or a meniscal tear, and — critically — why your MRI is almost certainly showing findings that are pointing your care in the wrong direction. Plus the complete treatment progression: from unloaded to loaded extension and flexion mobilizations, and why the more arthritic your knee looks on imaging, the more likely flexion is the answer. What you'll learn: ✔ What a Knee Derangement is and its defining clinical feature — Directional Preference ✔ The 7 symptoms of a Knee Derangement that mimic arthritis and meniscal tear exactly ✔ Why MRI findings in people over 40 are almost always misleading — and the research that proves it ✔ The Rosedale et al. 2014 RCT: 40% of knee OA patients referred for joint replacement had a Derangement ✔ Why patients with the MOST severe arthritis on imaging had MORE Derangements than those with mild changes ✔ The 2024 RCT: pain from mean 6.1 to 2.9 in 10 weeks with Directional Preference exercises ✔ The 60/40 Flexion/Extension split — and why arthritic knees often respond best to flexion loading ✔ The full treatment progression: unloaded extension, loaded extension, unloaded flexion, loaded flexion ✔ Two real patient cases — same anterior knee pain location, completely different classification, completely different treatment Research referenced: Rosedale R et al. 2014 JOSPT (MDT RCT in knee OA — 40% Derangement, Ottawa Panel "strongly recommended"); Silva DR et al. 2024 (directional preference RCT — pain 6.1→2.9 at 10 weeks, 68% knee Derangement, 32% lumbar Derangement); Englund M et al. 2008 NEJM (35–56% meniscal tears on MRI in asymptomatic adults over 50); McKenzie Institute International Part E Extremities Manual 2026 (60/40 flexion/extension prevalence, force progression data). 🎁 FREE KNEE PAIN REPORT — Why Your Knee Still Hurts...
