Globally, the cost associated with medication errors has been estimated at $42 billion USD annually. Errors can occur at different stages of the medication use process.   • Lea Dias is the founder of a healthcare startup called Quaefacta. She is a Clinical Pharmacist by background and in the past worked as the CLinical Safety Pharmacist at the Perth Children’s Hospital.
