This simple question - that we health professionals should ask of all our patients - is at the heart of the Personalised Care Podcast.

It’s a new series, brought to you by the Personalised Care Institute, the national organisation which sets the standards for evidence-based personalised care training and is aimed at all healthcare professionals.

With our host Dr Bogdan Chiva Giurca, we’ll discuss how a personalised care approach – reframing the focus from ‘what’s the matter with you?’ to ‘what matters to you?’ - can be applied in different scenarios. We’ll explore the benefits of empowering patients to get more involved in decisions about their care, including better health outcomes, improved compliance with advice and more satisfied people, carers and healthcare professionals.

    In this special final episode, we focus on the role of personalised care in keeping people healthy within their homes and local community. 

    We explore this topic through the lens of NHS @home, a national programme aiming to ensure people have faster access to more appropriate and targeted care within the local community and at home without necessarily having to attend emergency care or arrange GP  appointments.

    In this episode we’re exploring the importance of embedding personalised care throughout undergraduate and postgraduate education, training and assessment.

    We’ll be looking at common barriers faced by health and care professionals when it comes to delivering personalised care under pressure during daily clinical commitments as well as exploring our own sphere of influence as formal and informal role models for the future generation of healthcare professionals.

    The Covid-19 pandemic has exacerbated and has brought pre-existing health inequalities into sharp focus. But what can we learn from the last 2 years and how can health professionals close the health gap? What can healthcare professionals do when individuals facing greatest deprivation are not only experiencing higher risk of exposure to the virus whilst already living with existing poor health and multiple comorbidities?

    Through a series of practical examples and case studies showcased by our guests, this episode explores the role of personalised care in reducing health inequalities through shared decision making, multidisciplinary collaboration, local community connections, patient engagement and patient empowerment.

    Long Covid and Personalised Care

    Over a year-and-a-half has passed since Covid 19 is thought to have arrived in the UK and it’s believed that nearly a million people are still suffering from the effects of Long Covid, a condition also known as Post Covid Syndrome. And, with such a variety of symptoms in such a new illness, treating the condition is far from straightforward.

    So how could a personalised approach to care help people suffering with the condition?

    Nuffield Health has created a rehabilitation programme encompassing patient involvement, participation and co-design.

    Davina Deniszczyc, a GP and Medical Director of Nuffield Health and Marc Holl, Head of Primary Care at Nuffield Health chat to our host and Personalised Care Institute ambassador, Bogdan Chiva Giurca.

    During this episode, Long Covid is the term used to discuss Post Covid Syndrome. Both are recognised by the NHS.

    It’s estimated that there are 2.5 million veterans living in the UK but they don’t always get the support they need from the medical profession.
