Caring through connection

A common sense approach to proving quality care, meeting patient needs based on a patient-centered approach, and reducing healthcare waste. As a healthcare consumer, I am seeking this value based geriatric primary care physician in Tampa Bay. Grateful to know others find value in providing this type of care. Yes, discharge to home and provide home care visits with paramedics to reduce unnecessary ER visits!
