"Respond to a private residence for a 2-year-old female with an altered mental status, history of PDCD" 

A history of what? You know... PDCD! 

During this episode we explore Pyruvate Dehydrogenase Complex Deficiency, or PDCD. PDCD is a genetic abnormality which affects the conversion of pyruvate to Acetyl Coenzyme-A in the Krebs Cycle. This abnormality prevents mitochondrial energy utilization, leading to neurological degeneration.

EMS providers often think about the acute care medicine component of their career. Although this is incredibly important, have you ever stopped to think about what is actually happening from an interpersonal perspective? By providing high quality, evidence supported patient care, we are providing a service to our communities and our patients which we ultimately bill and/or receive funding for. Because of this, patients are our customers, and deserve to be treated as such. 

As EMS professionals we need to be approaching EMS in a way that is clinically center, but driven by customer service, or as Will Guidara says, "Unreasonable Hospitality."

Often as EMS providers we are called to the "fall" or even more mundane sounding, the dreaded "lift assist." We arrive, we help the patient up, and on our way we go. But when was the last time you stopped to think about how the patient ended up on the floor in the first place? Do some patients just trip over the cat? Certainly. But often, our fall patients are sicker than we perceive, and need our help to facilitate their care. On a daily basis, EMS clinicians utilize pulse oximetry to measure the percentage of hemoglobin bound to oxygen when evaluating respiratory adequacy, But have you ever had a need to measure a patient's carboxyhemoglobin?
