Disrupting Sexual Health with Dr. Rachel Rubin

Dr. Rachel Rubin is changing the landscape of women’s health—and she’s not waiting around for permission. In this fiery conversation, we talk about why sexual medicine has been ignored for so long, the massive education gap around menopause and hormones, and Rachel’s work fighting to remove the FDA boxed warning from vaginal estrogen. We also get into why pelvic PTs and OTs need to own our role in recognizing hormonal and vulvar changes, why the future of women’s healthcare requires a multidisciplinary “pit crew,” and Rachel’s philosophy that no grown-ups are coming to save the day. Plus, we talk disruption, entrepreneurship, taking action, dealing with criticism…and why Rachel believes pelvic rehab providers are becoming leaders in this space. About Dr. Rachel Rubin Dr. Rachel Rubin is a board-certified urologist and sexual medicine specialist, nationally recognized for her work in women’s sexual health, menopause, hormones, and Genitourinary Syndrome of Menopause (GSM). Make the Business Work For You - Financial Success

When you leave a job to start your own practice, you're making a pretty massive trade: You're trading consistency for upside. Gone is the predictable paycheck, the regular schedule, the structure someone else created for you, and the relative certainty of knowing what you're going to make. In exchange? You get upside. And in this first episode of our Making Your Business Work for You series, we're talking about the most obvious kind: money. Because we all know the fantasy math. "If I charge $200 and see 40 patients a week…" And suddenly you're mentally spending the fortune you're obviously going to make as a practice owner. Except…that's not how this works. There are plenty of practice owners making less than they could working for someone else. We'll share some real numbers, break down what different caseloads actually mean for your take-home income, and talk about the financial ceiling of the solo model versus the very different challenges (and opportunities) of growing beyond yourself. We also break down the only four ways to make more money in your practice: Get more leadsConvert more of those leads into patientsKeep your patients longerRaise your pricesBut making more isn't the whole story. You can have a thriving practice and still panic over every cancellation, obsess over every eval who doesn't book, and wonder whether the whole thing is about to come crashing down after one slow week. So we're also talking about financial confidence—knowing your numbers, understanding the normal ups and downs of business, and being confident you're on pace to hit your goals. Because ultimately, running a small business is a get-rich-slow scheme. You stack the wins. You make an extra $25,000 this year. You invest it. You get smarter about taxes. You improve the business. You let those gains compound over years and decades. Your practice might be the single greatest wealth-generating machine you ever create. Just don't expect it to happen overnight. You Know His Research. But Do You Know Paul Hodges?

You might not immediately recognize the name Paul Hodges. But we can almost guarantee you've been taught his work. And if you're attending PelviCon (either in-person or with our Live Virtual option) you'll get the chance to learn from him directly! If you've learned that... 👉 The pelvic floor and diaphragm work together 👉 The pelvic floor is part of a larger pressure-management system 👉 Timing and coordination matter—not just strength 👉 The abdominals and pelvic floor don't function independently 👉 Intra-abdominal pressure isn't inherently bad 👉 Pain is far more complicated than simply "tissue vs. brain" ...then you've been influenced by research Professor Paul Hodges and his collaborators have spent decades developing. And that's why we were SO excited to sit down with him.  Paul is one of the most influential physiotherapy researchers in the world, and in this episode we get to nerd out with him about the science that has fundamentally shaped how we practice. We talk about: 🔬 What we got wrong about his early motor-control research — and how complex science gets oversimplified when it reaches the clinic. 💨 Why pressure isn't the enemy. Paul introduces the Swedish concept of lagom: we need the appropriate amount of pressure for the task—not simply less pressure. ♂️ Why we can't just extrapolate female pelvic floor rehab to men. Paul's team has been investigating what actually creates continence after prostatectomy and how that should change rehabilitation. 🧠 Why we've oversimplified chronic pain. We dig into nociceptive, neuropathic and nociplastic pain—and why chronic pain doesn't automatically mean the tissues are irrelevant. 🤯 The problems with “central sensitization.” Including Nicole getting a little surprise when Paul explains the limitations of the Central Sensitization Inventory! 📚 How research actually makes its way into clinical practice—and why Paul believes research should ultimately change outcomes for real people. And maybe our favorite theme of the entire conversation: Complexity doesn't mean we throw our hands up. Our job is to understand that complexity well enough to find the clinically meaningful pieces that help the person sitting in front of us. 🎟️ Learn From Paul LIVE at PelviCon 2026! If this conversation gets your clinical brain firing, just wait until PelviCon.  (You can still get your tickets at www.pelvicon.com/virtual) Paul is giving two presentations during the main PelviCon conference: Beyond the Squeeze: Ultrasound and Precision in Male Pelvic Floor Care and Same Symptoms, Different Pain: Phenotyping Pelvic Pain in Clinical Practice Your PelviCon Live Virtual + Recording ticket gives you access to Paul's presentations PLUS all 14 talks from our eight incredible speakers—and you have lifetime access to the recordings. His work spans decades and hundreds of scientific publications, but what makes it so relevant to us is how many of the principles we now take for granted in pelvic rehab have roots in questions Paul and his collaborators were asking long before those concepts became commonplace.  📱 Follow Paul on Instagram: @paulwhodges 🔬 Learn more about his research and academic work: Professor Paul Hodges — University of Queensland About Nicole & Jesse Cozean Pelvic health is hard. Do You Actually Need a CRM? Fight for Simplicity.

When does a cash-based pelvic health practice actually need a fancy CRM? For most of us? Never. And yes—we've made this mistake ourselves. 😬 In this episode, we're breaking down what a CRM actually is, how it's different from your EMR and email platform, and why adding more technology often creates more problems instead of solving them.  We get into: 👉 Why you probably already have a CRM (even if it's just a Google Sheet!) 👉 The difference between your CRM, EMR and email platform 👉 Why PelvicSanity went from a simple Call Log → fancy CRM → right back to the Call Log 👉 The hidden costs of tech stacking and automation 👉 Why you shouldn't build your business systems around a specific admin 👉 The question you should ask before adding ANY new software, system or complexity The bigger principle? Fight for simplicity. Before adding something new to your business, get crystal clear:  What problem am I actually trying to solve? Then ask whether this is really the simplest, best way to solve it. Because in more than 1,000 pelvic health business owners we've worked with, we've never once thought: "You know what's really holding this business back? They need a better CRM." 😆  🚀 Want Our Actual Systems? Join the Accelerator! This is exactly the kind of stuff we work through in the Pelvic Health Business Accelerator. You don't need to reinvent your systems or spend months testing fancy software. We give you the actual tools, systems and frameworks we've built and refined at PelvicSanity—including the Call Log we still use today—and help you implement them in your own practice.  If you're ready to stop guessing and build a simpler, more profitable pelvic health practice: 👉 Check out the Pelvic Health Business Accelerator (www.pelvicptrising.com/accelerator) 🎟️ And PelviCon Is Coming! Maternal Mortality, a Whole-Body Approach and Using Data: An Interview with Katherine Sylvester

How can we help save the lives of our pregnant and post-partum patients?  What about doing more than just resolving their reported symptoms? We can't wait for you to hear this conversation with Katherine Sylvester - she has spent her entire career challenging the status quo and pushing for better results for her patients.   After experiencing postpartum preeclampsia herself, Katherine began asking how wearable data could help women recognize problems earlier, advocate for themselves and make better decisions during pregnancy and postpartum. That eventually grew into Operation M.I.S.T. and her work helping women use their own data to better understand their bodies.  And that's just one part of this conversation! We talk about: 👉 How Katherine's own postpartum experience led to Operation M.I.S.T. 👉 Using wearable data to identify concerning changes before they become major problems 👉 Teaching women to self-monitor and make data-driven decisions 👉 Why postpartum return to exercise needs to be individualized 👉 Her mission to reduce unnecessary C-sections 👉 Why we're barely scratching the surface of what pelvic PT/OT can do with scar management 👉 What happens when you stop complaining about problems and start asking, “What can I do about this?” Katherine is bringing two completely new topics to the PelviCon stage this year: using data to address maternal mortality and better understanding and treating scars in pelvic rehab. We can't wait for you to learn from her!  🎟️ Join Us for PelviCon 2026 — From Anywhere! Can't be with us in Atlanta? You can still experience PelviCon LIVE. The Hardest Part of Hiring Your First Clinician

What if the first clinician you hire isn't as good as you? It's one of the biggest fears we hear from successful solopreneurs—and it's exactly what Nicole struggled with when we started growing PelvicSanity. Because hiring your first clinician isn't just adding another person to the schedule. It's changing your entire role. Suddenly, you have to trust someone else with your patients, your reputation, your culture and something you've worked incredibly hard to build. And yes...Nicole struggled with this enough that she eventually went to therapy to work through some of the control issues that came with becoming a leader. 😬 In this episode, we're talking about the mindset shifts required to successfully go from solopreneur to multi-clinician owner, including: 👉 Why being busy isn't a good enough reason to hire 👉 "No one will ever be as good as me" (and what to believe instead) 👉 Why your next clinician can actually enhance your reputation 👉 Raising the floor with great systems, onboarding and mentorship 👉 Raising the ceiling by helping your clinicians become incredible 👉 Why your clinicians don't need to be perfect on Day 1 👉 The fear that you'll invest in someone...and they'll eventually leave Ultimately, the traits that made you a successful solopreneur—control, high standards, personal responsibility—can become the exact things that prevent you from building a great team. Growing beyond yourself requires becoming a different kind of business owner. 🎧 Listen wherever you get your podcasts! Want to Grow as a Business Owner? The Biggest Mistake We Make with Downtraining

If you've ever felt like you've "tried downtraining" and your patient still isn't getting better... this episode is for you. We're also wrapping up our recent series on internal exams, the gold standard, and access to quality care before shifting back into one of our favorite clinical topics: downtraining. The biggest mistake we see? Thinking of downtraining as a single intervention. In this episode, we introduce the updated framework from our completely re-recorded Downtraining Masterclass, including the three different "levers" of downtraining and how they help you identify why someone's pelvic floor is overactive—not just that it is. In this episode we discuss: ✅ A quick recap of our recent series on internal exams & the gold standard ✅ Why "I've tried downtraining" usually isn't true ✅ The 3 Downtraining Levers:  Pelvic Floor Direct  Pelvic Floor Indirect  Nervous System ✅ How to choose the right intervention for the right patient ✅ When to stay the course... and when it's time to pivot If you treat patients with pelvic pain, overactivity, urinary urgency, constipation, or other pelvic floor dysfunction, this framework will immediately change how you think about treatment. 🎓 Ready to Go Deeper?
