    On this 13th and last episode of the Value-based healthcare podcast I spoke with Joe Casey whom I have known for years from the EUHA (European University Hospital Alliance). He is director of partnerships and programs for King's Health Partners and he has a ton of experience in implementing VBHC. We cover a lot of topics, from the breadth of his work within the system that is King's Health Partners, about how the pandemic changed the focus of his work, to his general view on Value-based healthcare which I want to quote "VBHC is not a strategy that will fix healthcare, there is not strategy that will fix healthcare. VBHC is a framework that supports people to talk, to understand one another to understand their differences". The book he recommends is "Radical Uncertainty" by John Kay Mervyn King.     Frau Hecker ist Ärztin und Chief Patient Safety Officer am UK Essen, engagiert beim Actionsbündnis Patientensicherheit (APS) und bei Vision Zero Oncology  Das UK Essen ist die einzige Uniklinik, die diese Position aktuell besetzt hat. Ihre Aufgabe ist, dafür zu sorgen, dass in Unternehmensentscheidungen das Thema Patientensicherheit als Faktor mit einfließt (beschrieben ist die Position hier im “APS Weißbuch Patientensicherheit” von M. Schrappe https://www.aps-ev.de/aps-weissbuch/) Inspiriert  von Dr. Zimlichman vom Sheba Hospital sieht sie es als ihr Mantra zu sagen “wir sind Ärzte, wir machen Fehler, und manchmal passieren aus diesen Fehlern auch Patientenschäden” Wir sprechen kurz über CIRS (Critical Incident Reporting System). Was das ist, wird hier erklärt  https://de.wikipedia.org/wiki/Critical_Incident_Reporting_System, Frau Hecker berichtet, dass dies weiter zu wenig eingesetzt wird und eine gute Nutzung nicht vorhanden ist Qualität ist nicht gleich Patientensicherheit  Wir sprechen über den Unterschiede in der Haltung zu Fehlern zwischen Ärzten und Piloten, wir reden über das Thema Checkliste. Und meistens geht es gar nicht um fachliche Fehler, sondern um fehlende Kommunikation um das nicht- Einhalten von Prozessen Wir kommen dann zu PROMs und Patientensicherheit, wir reden zunächst über Prozessschritte: Input, Throughput, Output. Was hier dann noch sich anschließt ist das Outcome. Dazu sind die PROMs essentiell wichtig.     In this podcast episode, I speak with Ali Hashemi, a true visionary and serial entrepreneur: He talks about founding Metabolic, after selling his previous company. He then took time to reimagine healthcare, emphasizing the transformation of data from wearables into actionable health outcomes. Metabolic offers a hybrid care model for diabetics, combining standard physical care based on guidelines with a tech-enabled approach. Initially, technology was deemed crucial, but it became clear that human interaction in healthcare is even more impactful. A key aspect of Metabolic's service is the comprehensive care journey for diabetics, including blood tests and physical check-ups in a single visit, with immediate discussion of results. This approach allows for in-depth patient education and understanding of their health data, a process they term “transfer of agency.” This empowers patients to make informed health decisions. Metabolic’s care model extends beyond standard practices, including a value-based reimbursement component and a risk-sharing approach, initially offering a "satisfied or money back" guarantee. This innovative model challenges traditional insurance structures, especially for chronic conditions like diabetes, which don't align well with conventional insurance that covers catastrophic, low-probability events. We then speak about the inefficiency of forcing chronic care expenditure through health insurance, which often leads to non-value-adding expenses. This is particularly problematic in systems like Dubai's, where insurance is based on yearly renewable contracts. The episode also explores diabetes care for younger populations and other chronic conditions like Graves' disease. Ali then recommends the book “Outlive” by Peter Attia, which aligns with the theme of proactive and data-driven healthcare management.
