Skin Cancer Biopsy: Why Getting the Right Biopsy Matters - with Dr Adam Greenbaum

When a suspicious skin lesion needs further investigation, a biopsy can provide important information - but not every biopsy is the same. Should a small sample be taken, or should the entire lesion be removed? And why can that decision matter when planning treatment for a suspected skin cancer? In this episode of the Cutting Edge Plastic Surgery Podcast, Specialist Plastic and Reconstructive Surgeon Dr Adam Greenbaum explains how biopsies are used in the diagnosis and treatment planning of skin cancer. He explains the difference between a punch biopsy and an excision biopsy, using the simple analogy of an apple core or soil sample to demonstrate the advantages - and limitations - of examining only a representative portion of a lesion. The type of biopsy selected can also differ depending on whether the suspected lesion is a basal cell carcinoma, squamous cell carcinoma or melanoma. During the discussion, Dr Greenbaum covers: ·       What doctors actually mean by "biopsy" ·       Punch biopsy versus excision biopsy ·       Why sampling a lesion can sometimes be appropriate ·       Why removing the whole lesion is preferable in other circumstances ·       How biopsy findings can help determine the extent of surgery required ·       The approach to basal cell carcinoma ·       The approach to squamous cell carcinoma ·       Why suspected melanoma requires particular consideration ·       What a minimal-margin excision biopsy is ·       Why melanoma depth matters when planning subsequent surgery One of the key messages is that a biopsy isn't simply a test for cancer. The information obtained can help establish the diagnosis, determine the characteristics of a tumour and guide what treatment may be appropriate next. Understanding why a particular biopsy has been recommended can therefore be an important part of understanding your overall treatment plan. This video provides general educational information and is not medical advice. If you are considering surgery, please consult a qualified healthcare professional. Gynaecomastia, commonly referred to as enlarged male breast tissue, can affect men of different ages and may influence comfort, confidence, and clothing fit. In this episode of the Cutting Edge Plastic Surgery (CEPS) Podcast, Specialist Plastic Surgeon Dr. Adam Greenbaum discusses Gynaecomastia, including common causes, treatment considerations, surgical options, and factors patients may wish to understand when exploring male chest reduction surgery. This discussion is intended to provide balanced, educational insights for individuals seeking to better understand Gynaecomastia and the options that may be available following professional assessment. In this episode: • What Gynaecomastia is and common contributing factors? • How Gynaecomastia is assessed clinically? • Non-surgical and surgical treatment considerations • What male chest reduction surgery may involve? • General recovery and healing expectations • The importance of specialist assessment and personalised treatment planning Every patient’s anatomy, goals, and circumstances are unique. A comprehensive consultation with a qualified medical professional is important when considering any surgical procedure. Depending on the cause and severity, there may be non-surgical approaches for Gynaecomastia, although these are not suitable for everyone. Non-Surgical Options May Include: • Addressing underlying causes such as medication use, hormonal imbalance, weight changes, or certain health conditions  • Weight management and exercise where excess fatty tissue is contributing  • Monitoring mild or temporary cases, particularly during puberty  • Medical assessment and investigation where hormonal factors are suspected  However, when there is: • Persistent glandular breast tissue  • Significant excess tissue or skin  • Long-standing Gynaecomastia  • Limited improvement with lifestyle changes  …surgery may sometimes be considered following specialist assessment. In many cases, an evaluation is important to determine whether the enlargement is: • Fatty tissue  • Glandular tissue  • Or a combination of both  This helps guide appropriate treatment options. For anyone concerned about Gynaecomastia, the best first step is a consultation with a qualified medical professional to understand the underlying cause and discuss suitable management options.
