    106. What Happens When You do not Listen to Your Leaders

Roots of Change Podcast — Episode 106 — What Happens When You Do Not Listen to Your Leaders An experienced nurse took a management position with genuine commitment, clinical expertise, and practical ideas for improving patient safety on her unit. She raised her concerns. She offered solutions. She kept trying. The response was always the same. We don't do things like that here. In this episode, Diane Gudmundson shares that story, one side of a real situation, because she believes that if the leaders above her had taken time to genuinely hear that perspective, things may have turned out very differently for everyone involved, including the patients. This episode is about what organizations lose when they stop listening to the people closest to the work. Not just good people. Their most important early warning system. And the cost that eventually reaches the patients at the end of the chain. Diane also shares what the opposite looks like from her own experience of leading from a genuinely people-centred place, and what patient-centred leadership at the senior level actually requires. If you are a senior healthcare leader and this resonates with your organization, send Diane a direct message with the word ROOTS and she will share the next step. When we stop listening to them, we do not just lose good people. We lose our best chance to catch problems before they reach the patients.

    105. Moving from Clinician to Leader, what noone tells you!

Roots of Change Podcast - Episode 105 - Nobody Told Me This Would Happen There is a moment most clinicians who move into leadership remember clearly. The day the dynamic shifted. The day the skills that made them exceptional at the bedside stopped being enough on their own. In this episode, Diane Gudmundson identifies two patterns she has consistently observed when clinicians transition into leadership. The first is the clinician who defaults to overgiving, making herself available for everything, absorbing everyone's problems, trying so hard to be liked that she gradually loses the team's respect without understanding why. The second is the clinician who defaults to control the moment the title arrives, micromanaging, performing authority, more focused on how she looks to her own director than on the people she is actually responsible for. Neither pattern comes from bad intentions. Both stem from walking into a new role without a map and defaulting to what was learned elsewhere before leadership ever began. Diane also names what clinical training genuinely gives a new leader: the ability to listen, to read a room, to care deeply, to see the human being behind the role, and what it does not give: the candour, decisiveness, and accountability that leadership also requires. This episode is for any clinician thinking about stepping into leadership, who just made the transition, or who has been leading for a while and still finds it harder than expected. 104. They Left Healthcare. The Wisdom Left With Them

Roots of Change Podcast - Episode 104 - Where Does the Wisdom Go? A question came to Diane Gudmundson recently that she has not been able to set aside. Where does all the wisdom go when really excellent professionals leave healthcare? In this episode, Diane shares what she has been hearing consistently from nurses, nurse practitioners, and experienced healthcare professionals who left the system. Not because they stopped caring, but because the conditions inside the system made it impossible to be the version of themselves they wanted to be for patients. Micromanaged. Restricted from working to full scope. Watching management focus on budgets and policies while the patients got further from the centre of the work. Diane shares her own experience of leaving the public healthcare system to build NP on Wheels, and what she realized when she started talking to others who had made similar decisions. Healthcare counts vacancies and tracks turnover. It has no way of measuring what experienced people take with them when they go. The pattern recognition. The relationships. The deep understanding of where the system fails the most vulnerable and why. This episode introduces the Roots of Change Creative Conversation, a small group of 10 to 12 experienced healthcare professionals/leaders coming together over three sessions, starting August 22, to explore one question. Is there an unmet need for a credible way for experienced healthcare people to use their knowledge and experience to contribute to healthcare change? And if there is, what would actually help? If you have spent years in healthcare and still believe there is a better way, Diane would love to hear from you. Send a direct message with a brief note about your healthcare background and why this conversation interests you. The group is limited to ten to twelve people. That wisdom is still out there. So what are we going to do about it?

    103. Why is it "Lonely at the Top" of a hierarchy?

Roots of Change Podcast — Episode 103 — Why It's Lonely at the Top When Diane Gudmundson asks senior healthcare leaders what the hardest part of their job is, two answers keep coming back consistently. Team accountability and loneliness. In this episode, she shares her own leadership arc, from absorbing an authoritarian model growing up, to struggling through her first nursing leadership role and wondering if she was simply not meant for leadership, to working for a patient-centred leader early in her NP career who showed her a completely different way was possible. She traces what happened through the years of controlling, passive, and passive-aggressive leaders that followed, through building NP on Wheels, and through her grassroots contract in Manitoba where she experienced firsthand that leading around people instead of reports and policies meant she never felt lonely once. She also explains why hierarchy itself produces isolation at the top through a mechanism most leaders never examine, the way information gets shaped at every level before it reaches the person making the decisions. And she shares what actually changes when a leader is genuinely connected to the ground. If you are a senior healthcare leader and this resonates with your organization, send Diane a direct message with the word ROOTS and she will share the next step. 102. Symptoms Visible but the Truth is Hidden

Roots of Change Podcast Episode 102: When the Symptoms Are Visible but the Truth Is Hidden A healthcare executive had been told for months that a rural facility couldn't keep staff because of the labour market. When Diane Gudmundson went directly to the community, she found something completely different. There were people available. They just wouldn't apply. Because in a small town, word travels. And the word about that workplace had been travelling for a long time. In this episode, Diane walks through what she actually found on the ground, how that information was altered at every level before it reached the executive, and what it reveals about one of the most dangerous gaps in healthcare leadership. The gap between what the data shows and what is actually happening with the people inside and around an organization. She also walks through the specific questions patient-centred leaders ask differently, and why going to the ground is not a soft leadership approach. It is the most sophisticated intelligence work a healthcare executive can do. If you are a senior healthcare leader and this resonates with your organization, send Diane a direct message with the word ROOTS, and she will share the next step. 101. The Advocate who Needed One

Roots of Change Podcast — Episode 101 — The Advocate who needed one.... I spent twenty five years fighting for patients the system had decided were too complicated, too risky, or simply too inconvenient to deal with. And then the system came for me. In this episode, Diane Gudmundson tells the story she has never told quite this directly before. After decades of fierce patient advocacy, she found herself on the receiving end of a regulatory authority that demanded psychiatric assessments, skill assessments, and multiple courses, then ignored every reference, every assessment result, and every piece of evidence she submitted. The investigations kept coming, each one more disconnected from reality than the last, until she understood clearly that this was no longer about truth. It was about a system using power to silence the person who had been most loudly advocating for the people it was failing. This episode is about what happens when power stops listening and starts protecting itself. It is about the cost of speaking up in systems that prefer quiet. And it is about why that cost is never a reason to stop advocating for the people who need it most. Because you cannot build a patient-centred system on a foundation that punishes the people who are most patient-centred. 100. The FIRE in me that never went out!

This is episode 100. And it is the episode Diane Gudmundson has been building toward for thirty years. In this milestone episode, Diane tells the complete origin story behind the Roots of Change doctrine, the method, and the movement. She shares what she witnessed across thirty years as a nurse and nurse practitioner working from the ground up, including a woman dying in pain because a system everyone called excellent had left a gap nobody was tracking, clinics turning patients away because they were too complicated, families left without support in their most desperate hours, and what she perceived to be a regulatory authority making decisions that resulted in hundreds of vulnerable patients being abandoned. She shares the fury that lit a fire in her that has never gone out. The decade she spent searching for the patient-centred leadership she experienced in her very first nursing job and never found again. The moment she walked away from the system to find the root. And why she believes that LISTEN is not a soft skill. It is the only thing that will transform healthcare from the inside out. This episode is the origin story of everything. The book, the doctrine, the method, the movement, and the conviction that power must be responsible enough to protect the people with less of it. 99. Patient-centred leadership has nothing to do with being liked

Roots of Change Podcast: Patient-Centred Leadership Has Nothing to Do With Being Liked A formal complaint from a patient's family landed on a leader's desk. He read it. And then he did nothing. Not because he missed it. Because confronting the staff member involved felt harder than looking the other way. In this episode, Diane Gudmundson shares what it felt like to watch that unfold as a member of that team, what it cost the people who were holding themselves to standards every single day, and what it revealed about one of the most misunderstood patterns in healthcare leadership: the difference between being liked and being safe to work for. She also names the superpower every clinician who became a leader already has and most have stopped using. The ability to deliver hard news with care. To hold someone accountable without abandoning them. To have the conversation nobody wants to have because the patient's dignity depends on it. Patient-centred leadership is not about keeping everyone comfortable. It is about having the courage to protect the people counting on you, including the ones watching to see what you do next.
