Joy Jang 如果性交已經不痛了,治療就結束了嗎? 在上一集,我和美國骨盆健康治療師 Joy 談到女性性交疼痛:疼痛可能來自組織、荷爾蒙、骨盆底肌、神經系統,也可能與恐懼、過去的經驗及親密關係交織在一起。 這一集,我們想把問題再往前推一步: 當疼痛逐漸改善之後呢? 在醫療與復健中,我們很習慣用「症狀消失」衡量治療是否成功。 不痛了。 肌肉放鬆了。 功能恢復了。 但對性健康而言,沒有疼痛,不等於擁有愉悅。 一個人可能已經可以接受觸碰或進行性交,卻仍然感到緊張、沒有感覺,甚至只是覺得「至少可以忍受」。 如果治療停在這裡,真的算恢復了嗎? 這一集,我和 Joy 從骨盆復健的角度,繼續談性交疼痛之後更少被討論的一端——愉悅。 Joy 在對談中也提到一個我很喜歡的觀點:性健康並沒有年齡限制。 我們常常談年輕人的性健康,卻很少談到高齡者的性健康。年齡增長並不代表親密、性與愉悅就應該從生命中消失;高齡者同樣可以擁有健康的性生活,也有追求性愉悅的權利。 而性愉悅也不只有性交這一種形式。認識自己的身體、觸碰、自慰,以及與伴侶之間的親密互動,都可能是性健康的一部分。 這也讓我們重新思考:骨盆復健的目標到底是什麼? 骨盆底肌並不是越緊、越有力越好。它需要有力量,也需要能夠放鬆;需要能收縮,也需要在適當的時候延展,並與呼吸、軀幹、髖部及全身動作協調。 而性功能,也不只是某一塊肌肉的功能。 感覺、性喚起、潤滑、疼痛、動作、情緒、大腦與神經系統,都可能參與其中。 因此,骨盆復健不應該只問: 「現在還會不會痛?」 我們也可以開始問: 「妳覺得舒服嗎?」 「妳能感覺並重新連結自己的身體嗎?」 「妳可以放鬆,也可以主動控制自己的骨盆底嗎?」 「親密接觸對妳而言,能不能重新成為一件可以期待的事?」 這也是我很想透過這一集傳達的觀念: 疼痛的反面,不只是「不痛」。 當我們談性健康,或許可以把目標放得更遠一點—— 從疼痛,到舒服、自在與連結,甚至重新找回愉悅。 因為健康不只是疾病或症狀的消失。 愉悅,也是健康的一部分。 #性交疼痛 #女性性健康 #骨盆健康 #骨盆復健 #骨盆治療 🎬 Beyond Silence: Cross-Cultural Conversations on Pelvic Health EP4 From Pain to Pleasure: Rethinking Painful Sex and Women’s Pelvic Health ft. Joy Jang If sex no longer hurts, is treatment finished? In our previous episode, pelvic health therapist Joy and I explored pain during sex—how tissues, hormones, pelvic floor muscles, the nervous system, fear, past experiences, and relationships can all contribute to pain. In this episode, we take the conversation one step further: What happens after the pain gets better? In medicine and rehabilitation, we often define success by the absence of symptoms. The pain is gone. The muscles have relaxed. Function has returned. But when it comes to sexual health, being pain-free is not the same as experiencing pleasure. Someone may be able to tolerate touch or intercourse again and still feel tense, disconnected, or simply think, “At least I can do it now.” Is that really where rehabilitation should end? In this episode, Joy and I explore a side of pelvic rehabilitation that receives far less attention: pleasure. Joy also shares an important perspective: sexual health has no age limit. We often talk about sexual health in younger people while rarely discussing sexual health in older adults. Growing older does not mean that intimacy, sexuality, and pleasure should disappear from life. Older adults can continue to have healthy sexual lives and have the right to pursue sexual pleasure. And sexual pleasure is not limited to intercourse. Understanding and exploring one’s own body, touch, masturbation, and intimacy with a partner can all be part of sexual health. This leads us to a broader question: What should the goal of pelvic rehabilitation really be? A healthy pelvic floor is not simply a strong or tight pelvic floor. It needs strength, but it also needs the ability to relax. It needs to contract when appropriate, lengthen when necessary, and coordinate with breathing, the trunk, the hips, and the rest of the body. Sexual function, likewise, is about much more than one group of muscles. Sensation, arousal, lubrication, pain, movement, emotion, the brain, and the nervous system can all play a role. So perhaps pelvic rehabilitation should not stop at asking: “Does it still hurt?” We can also begin asking: “Does it feel comfortable?” “Can you feel and reconnect with your body?” “Can you both relax and actively control your pelvic floor?” “Can intimacy become something you look forward to again?” That brings us to an idea at the heart of this episode: The opposite of pain is not simply “no pain.” When we talk about sexual health, perhaps we can aim further—from pain to comfort, ease, connection, and even pleasure.
