On this episode of Sugar Crush: The Rest of the Story, Dr. Rick Jacoby welcomes Grok into the studio for a deep dive into the fascinating world of stem cells, medicinal signaling cells, exosomes, nitric oxide, blood flow, and regenerative medicine. Dr. Rick begins by separating embryonic and non-embryonic stem cells and revisiting the history of the field, including the work of Arnold Caplan, who originally popularized the term mesenchymal stem cells before proposing that they instead be called medicinal signaling cells—reflecting the idea that much of their biological activity comes from the signals they send rather than simply transforming into replacement tissue.  So how do those signals work? The conversation goes microscopic, exploring pericytes, growth factors and exosomes—tiny extracellular vesicles that can carry proteins, RNA and other signaling molecules between cells. Dr. Rick also discusses experimental research involving exosomes and nitric oxide and why he believes improving circulation and oxygen delivery is fundamental to the body's healing processes.  That leads to another critical player: nitric oxide. The conversation starts with an observation at a hospital in Scottsdale, where signs warning against aggressive behavior seem to be everywhere. For Dr. Rick, they're more than security notices—they're a symbol of a healthcare system in which patients, providers, institutions and taxpayers are all feeling the pressure.  From there, he travels back to Benjamin Franklin, Dr. Thomas Bond and the founding of Pennsylvania Hospital in 1751, using America's early spirit of community involvement as inspiration for what he believes needs to happen today. His metaphor is Franklin's Bucket Brigade: when there was a fire, ordinary citizens didn't wait for someone else to solve the problem. They grabbed a bucket, passed it to the next person and worked together to put it out. Dr. Rick argues that it's time to do the same thing with health. He explains anterograde and retrograde transport and uses the image of a two-way highway inside every nerve to illustrate what happens when that system begins to break down.  From there, he explores glycation—his central theme that excess sugar can interfere with proteins and normal biological processes—and connects it to his larger theories surrounding diabetic neuropathy and nerve dysfunction. The conversation then moves to the electrical side of the nervous system: myelin, the nodes of Ranvier, small versus large nerve fibers, pain receptors, nerve-conduction testing, and the remarkable process that allows an electrical signal to race from your fingertip to your spinal cord and brain.  But the heart of the episode is Dr. Rick's description of the progression of diabetic neuropathy.
