    Ep125 Why “That’s How It’s Always Been Done” Is Costing You

    Most of our biggest limitations aren’t technical or physical, they’re simply habits we’ve stopped questioning. The time something takes, the way a process is structured, the number of people required, often these feel fixed only because nobody has challenged them in years.  Ishan traces that idea across three very different worlds. At Toyota, one engineer’s refusal to accept hours-long machine changeovers helped reshape an entire production system.     Ep124 What Maintenance Can Teach Us About Excellence – Ian Cooper

    Communication, not skill or technology, is the real bottleneck on most factory floors. That’s according to fitter-turned-business-owner Ian Cooper, who argues it’s lean manufacturing’s unspoken eighth waste: when maintenance and operations don’t understand each other’s constraints, Band-Aid fixes quietly become permanent and trust erodes.  Ian grew up inside the trade, but deliberately didn’t start there, choosing to complete his apprenticeship outside his father’s business so no one would see him as anything but another young guy in industry.     Ep123 Hidden Capacity: Stop Digging Holes That Don’t Need Digging

    Most organisations chase capacity in the wrong place. We add people, extend hours, buy more technology, when the capacity we're looking for is often already there, quietly trapped beneath rework, excess and delay that everyone has simply learned to accept as normal.  Lego found $450 million through a single turnaround not by producing more, but by stripping out unnecessary complexity. Aldi built its entire model around the same principle. And British Cycling's "marginal gains" philosophy shows why we should stop celebrating people for constantly compensating for broken systems, and start asking what's actually getting in their way.  Sometimes unlocking excellence isn't about squeezing more effort from your people.     Ep122 Converge, Diverge, Converge: The Rhythm of Great Problem Solving

    Different perspectives feel like a strength, until they quietly become the reason five people leave a meeting solving five different problems. A tapping exercise from Stanford reveals why: once you know something, you can't imagine not knowing it, and that gap is where teams get stuck.  The fix isn't choosing between alignment and diversity of thought. It's knowing which one the moment calls for. When defining the problem, everyone needs to see the same thing. When analysing why it's happening, difference becomes the asset, provided people feel safe enough to voice it. When solving it, keep exploring before settling.     Great problem solvers don't chase theories, they chase evidence, even when it takes years to find it. That's the lesson from Captain John Cox, who led the four-year investigation into USAir Flight 427, one of the most complex air crash investigations in history.  With 56 years in aviation, including 25 years flying for major airlines and over 14,000 flying hours, John built one of the world's most respected aviation safety consultancies, now in its 21st year. A veteran accident investigator and commentator on the TV series Air Crash Investigations, he's spent decades teaching organisations how to move from reactive fixes to proactive risk management.  In this episode, John walks Ishan through how he assembled and led an investigative team, why confirmation bias is one of the most dangerous traps in complex problem solving, and how filtering signal from noise in overwhelming data determines whether you find the real cause.     Ep119 Understanding the Problem Before Solving It

    Real problem solving starts long before the whiteboard comes out. When Flight 427 went down in 1994, it took investigators nearly five years to find the answer, not because they lacked evidence, but because premature theories are seductive and discipline is hard.  Most workplace problems aren't life or death, but the same trap catches us daily. We hear "productivity's down" or "customers are unhappy" and jump straight to solutions, skipping the one question that matters: what exactly is the problem?  Three disciplines cut through the noise. Define the problem with precision instead of reacting to a vague description of one. Bring in the people who actually live with the process, not just the ones in the meeting room. And focus energy on what's genuinely within your control, not what's merely worth complaining about.  Sometimes the fix isn't a better solution.     Ep118 Rework: How Small Errors Can Lead to Massive Problems

    Rework is the thief hiding in plain sight. We rarely count it as waste — we just call it "work." But every correction, resend, and re-check is a bill we already paid once, now due again.  A warehouse re-slotting project seemed to be working... until productivity refused to move. The real issue wasn't visible until pickers spoke up. That's misalignment in action — the same failure that cost Airbus 500km of rewiring and two lost years on the A380, because different teams were building to different specs.  Then there's timing: a 1.3mm error on Earth became a multi-year, billion-dollar fix once Hubble reached orbit. The lesson? Mistakes don't grow — the distance they travel before discovery does.  And when rework escapes the business entirely, customers inherit it, as Takata's global airbag recall proved at catastrophic scale.  Small rework isn't harmless.
