The Most Effective Ways to Prevent Falls, with Dr. Thomas Gill

Our series on falls and fall prevention continues with the expertise of Dr. Thomas Gill, of Yale University, a specialist in geriatrics and fall prevention. This episode is a good partner to the last one which focused on medication, so be sure to circle back to the conversation with Dr. Michael Steinman, if you haven't listened to it already! Today, we're going deeper into fall prevention, exercise, fall preparation and ultimately, what to do if (or when) you fall. Because we can't assume that it will never happen. Indeed, 30% of persons over the age of 65 will have at least one fall over the course of a year. About a third of these lead to some sort of injury, like a hip fracture. But we can absolutely do our best to avoid them, while staying prepared if it does happen. Dr. Thomas talks to us about the value of Tai Chi and yoga for improving flexibility and muscle strength, as well as preparing your home with good, solid furniture, support bars, and eliminating any obstacles that could trigger a fall, like loose rugs or those tables where the legs stick out. Who thought that was a good idea? And if you do fall, you need to know how to get up. That might be the hardest, and most dangerous, part of all. Knowing what to do is equally important knowledge for your partners, loved ones, or anyone who is on standby to help. Our thanks to Dr. Gill for his time and contribution to this important series! 0:00 Why fall prevention matters for older adults 4:13 Falls are not a normal part of aging 6:28 Common fall risks 11:15 Hip fractures and the serious consequences of a fall 13:42 Effective ways to prevent falls 17:15 Exercise, Tai Chi, yoga, strength, and balance 19:24 Preparing for a fall and protecting the head and hips 26:46 What to do after a fall and how to get up safely 29:56 Living alone, emergency support, and preventing long waits for help 32:13 Thoughts on independence and fall prevention Visit weareoldpodcast.com to learn more about Ken and the show!

"Today I Would Like to Talk About My Medications," with Dr. Michael Steinman

If there's any one thing that will help you live a long and healthy life, it's making sure you're prepared for falling. Note that we didn't say, "making sure you never fall." Falls happen, we can never prevent them one hundred percent. But what you can do is prepare yourself so that if you do fall, the impacts are as minimal as possible. Dr. Mark Steinman is a Geriatrician at the University of California, San Francisco, who generously shared his time to talk about the importance of both preventing and preparing for falls, and all kinds of useful, practical tips that can have a direct impact on your life and well-being. This means everything from looking at your current medication, what drugs to avoid, being proactive in talking about your medication and your history of falls, and of course the various safety hazards around the house. We may expect that our doctors will be right on top of all our medication and health history. But they typically have all of 30 seconds to review our chart, and are juggling who knows how many patients and cases per day. We need to come prepared to our appointments, ready to talk about our medication, ask specific questions, and help our doctors and nurses help us. Be proactive, doctors are super busy! Dr. Steinman emphasizes how by being proactive and prepared, you can greatly reduce the risk of falls, and if or when they do happen, minimize the chances of serious impact. Give yourself the tools to move through the day with confidence and safety! “The easiest way to think of medications that increase the risk of falls is any medication that changes a person's ability to clearly perceive and navigate the world.” 00:00 Why falls are a serious concern for older adults 02:17 How medications can increase fall risk 06:17 Why fatal falls may be increasing 08:40 Deprescribing: reviewing medicines safely 10:31 Sleep medications and over-the-counter risks 14:33 How to raise medication concerns with your doctor 22:58 Home safety, vision, strength, and balance 25:20 Walking, regular movement, and staying active 29:43 Closing message on reducing medication-related fall risk Visit weareoldpodcast.com to learn more about Ken and the show!

Gambling Traps Seniors Need to Watch Out For, with Dustin Gouker

Over the last few short years, it seems like not just gambling apps and ads have popped up everywhere, but that every thing can suddenly now be bet on. Even the weather, election outcomes, news events: Sounds like a problem, doesn't it? Our guest today, journalist Dustin Gouker, points out that gambling and betting have been around since time immemorial. People have and will continue to find ways to bet on anything. Dustin has spent years studying and writing about the issue, covering the rise of daily fantasy sports, legalized sports betting, and prediction markets. But, it's the access that has become far easier, and is thus making gambling more prevalent. It's a fine line between something that's done for fun and entertainment versus becoming a genuine problem and addiction. We go and spend $20 at the movies, or $50 to go to a sports event, purely to be entertained and enjoy ourself. So why is it any different watching that game at home, and spending $50 placing bets on various outcomes? You might even win some money! That's not going to happen when you pull out your wallet at Yankee Stadium! Ok, but clearly that $50 can turn into something far, far more. So today we're talking about the impact of sports betting and gambling opportunities these days, how to recognize warning signs, and in particular how seniors need to be aware of warning signs and gray areas that can hook you in, without you realizing it. Why Voluntourism is Not What You Think It Is, with Aisling Ledwith

It's a multibillion dollar, worldwide industry. Thousands of people, often seniors, go overseas to help in person with orphanages and other aid organizations. But what if it's not all it appears? Today we're pulling back the curtain on the dark side of orphanage tourism. What appears on the surface as a positive, hands-on experience to help out those in need, in fact typically has major structural issues, driven by malicious purposes. Aisling Ledwith, Senior Advocacy and Campaigns Officer at Lumos, is here to help shed light on the issue. Lumos presses governments to reform care systems and show how children can be safely reunited with families, instead of being left with often exploitive institutions. What are "institutions"? Basically another word for orphanages, but in fact, in an "orphanage," typically 80% of children do still have at least one living parent. The children end up there for all kinds of reasons, but cannot possible get the type of care and support that all children should have. And that's even if the institution has otherwise well-meaning intentions. Children need a proper caregiver, for one-on-one care and for attention. In other words, a parent. So what do we do if there is no parent? How you can use your skills, your money and your heart to make a difference without causing unintended harm? Aisling talks to us about what child trafficking is, and the frequent cases of exploiting children to attract donations. One major red flag: it shouldn't be easy to get access to children. It should be something where there are background checks and gateways in place, for their protection. So our takeaway today from this important and powerful conversation is, do your research, think about where your money is going, how you can best help, and how can we work towards strengthening the best outcome of all: that children have loving, committed, full-time individual caregivers to raise them as we all want to be raised. Why People Misread Your Face, with Dr. Leslie Zebrowitz

Have you ever thought about why you make a first, visual impression upon meeting someone? Whether it's a "baby face" or an "old mug," or any other stereotype, there's a lot more that results from that impression than just shrugging and moving on from it. People are treated differently based on their facial structure - we all are in one way or another - and there's a good chance that that impacts how you conduct yourself and go through life. My friend Dr. Leslie Zebrowitz is here to talk to us about the study of 'first impressions.' Do you have an advantage having a baby face? Are older adults treated differently? Is there a difference in first impressions between personality and looks? It's popularized in the media: the bad guys always look different from the good guys. Or conversely, you notice if a 'bad guy' looks surprisingly like a 'good guy,' or vice-versa. Now facial and age recognition does serve an instinctive, species-protective purpose: knowing what's a threat and what's not. A baby is less likely to attack you than a big, tough warrior-looking dude. But what about how criminal defendants are perceived in court, even by a judge? What impact does that make when they might appear innocent when guilty, or appearing guilty when in fact innocent? Plus there are other influences on how we create first impressions, from physical attributes to behavior, like how people walk, their gait, their eye focus and more. It's a fascinating conversation that really gets you thinking, and more aware of how you perceive those around you. And hopefully it will help you recognize those signs in yourself of instant assumptions and judgement, and have better interactions with people, better relationships and better appreciation for the human experience. Ironically(!), our guest's video wasn't working, so if you're on YouTube, you'll see this as an audio-only episode. But perhaps that in turn will help you focus on visualization and your own experience in judging a book by its cover - hey, we all do it! Create Your Digital Fortress, part 2 with Nora Duncan, AARP

Nora Duncan and I continue our discussion from last week about fraud, scams, security and keeping yourself safe from digital threats in the real world. Today, our focus is on giving you the tools to build your "digital fortress" - the wall around your various accounts that only you, or the most trusted people in your life, can access. This means things like two-factor authentication, biometric identities, avoiding debit cards and checks, being careful with services like Venmo and Zelle, and knowing how to use a credit freeze. Ultimately, we need to turn our perspective around and use the internet to our advantage when it comes to security. Programs and systems exist, and three minutes of inconvenience (to confirm security codes and checks) is much better than three years of trying to get your life back in order because of a scam or theft. Take some notes from today's episode, visit the AARP's excellent website on fraud prevention (aarp.org/fraud), and start laying the bricks for your own digital fortress!
