What Else Doesn’t Residency Teach Physicians (Things They Didn’t Teach Us in Residency, Part 2) Ep. 85

In this episode, Dr. Junaid returns to the idea of medicine's hidden second curriculum. He opens with the overlooked importance of protecting your own earning power through true own-occupation disability coverage, and moves into the abrupt, unsupported transition from trainee to supervisor. He then reframes early job-hopping as a normal, even healthy, part of professional development. The episode closes on a quieter, human note: the identity vacuum that appears once "trainee" stops being your answer to who you are, and the rebuilding required with partners, friends, and family who waited through the training years. 84: Why Longer Notes Are Less Useful (The Frankenstein Note)

In this episode, Dr. Junaid uses Mel Brooks' "Young Frankenstein" to examine how clinical notes become monsters stitched together through rampant copy-forward and copy-paste, as documented by UCSF and Penn studies. He walks through the dangers of carried-forward errors and how AMA E/M guideline changes shifted billing away from exhaustive templated documentation, meaning much of the "tuxedo" clinicians still add to notes is unnecessary padding rather than a requirement. He offers physicians and APPs three ways to reclaim their voice and points listeners in need of more support to the Charting Conquered Blueprint. 83: Physician Burnout and the Missing Feedback Loop

In this episode, Dr. Junaid explores why physicians can practice excellent medicine every day and still feel depleted, tracing the problem to a fundamental disconnect built into modern healthcare: you almost never find out how things turned out for the people you helped. Drawing on anthropology, organizational psychology, and studies from emergency medicine to radiology, he explains how the human brain was wired to refuel by witnessing outcomes. Dr. Junaid offers three practical strategies you can start this week to rebuild parts of the positive feedback loop: intentionally look up one patient outcome, keep a file of evidence that your work mattered, and close the loop for someone else by telling a colleague what you learned about how their care turned out. 82: In Defense Of Saber-Tooth Cats

In this episode, Dr. Junaid explores why your brain's evolutionary wiring is quietly breaking you in modern clinical practice, starting with the classic saber-toothed cat from every medical school stress lecture and the crucial frame that slide never shows: the all-clear. He unpacks two ancient survival systems and shows how medicine has sabotaged both by creating endless unresolved threats (open charts, possible dissections hiding behind reflux, patients who go home with no follow-up) and rewarding you for staying in the cave of bloated notes and broken workflows. He closes with three concrete prescriptions: build one hard all-clear into your day, close the chart before the next patient, and pick one small thing you've been avoiding and go check whether there's actually a saber-toothed cat outside or just the wind. For physicians and APPs ready to rebuild their clinical day so charting becomes a byproduct instead of a second shift, Dr. Junaid points to the Charting Conquered Blueprint at chartingconquered.com/blueprint. 81: The Weirdest Research of the Year

In this episode, Dr. Junaid takes a delightful detour from the usual pressures of clinical practice to explore the quirky, joyful side of science through the Ig Nobel Prizes, real research that first makes you laugh, then makes you think. He walks through some of this year's winners as well as some for the past, including a 1977 study on the aerodynamics of nose-blowing that every physician has unknowingly referenced, a psychology paper revealing why we love friends who are vicious to our enemies, and Italian research suggesting pizza might actually protect against illness, if it's made and eaten in Italy. This episode reminds us that reclaiming that sense of wonder, the kind that got us into medicine before the inbox took over, is worth protecting. This week's challenge: find the answer to one question you want to know, just because you want to know it, and let yourself have fun with it. 80: Let’s Turn the Tables - Measuring Admin Part 2

In this episode, Dr. Junaid continues his thought experiment from part one, exploring what happens when administrators miss the mark on decisions that affect clinical workflow, and reintroduces the concept of administrative harm, a term coined in the New England Journal in 2022 and studied across thirty-two organizations where only nine percent encouraged transparency in administrative decision-making. He introduces the click budget, a four-part standing policy that requires new required fields to be click-neutral, burden-estimated in advance, sunset-dated, and owner-assigned, emphasizing that physicians and APPs are the only sensors in the system and must measure and voice their own numbers to drive change. He closes by connecting this systemic work to the immediate, practical relief offered in Charting Conquered Blueprint, a self-paced program designed to help clinicians build workflows that make charting a byproduct of real-time decisions rather than a second shift. 79: Let’s Turn the Tables - What If We Measured Them

In this episode, Dr. Junaid introduces a thought experiment that starts with humor but quickly becomes serious: what if administrators were measured the way physicians and APPs are? Drawing on his dual role as a practicing med-peds physician and ambulatory IS medical director, he walks through a series of metrics all applied to a composite character he calls VP Jones. The exercise reveals a concept called administrative harm, defined in a 2022 New England Journal of Medicine article and studied across 32 organizations in JAMA Internal Medicine, where only 9% of participants said their organization encouraged transparency in administrative decision-making. Dr. Junaid argues the real problem isn't that administrators are bad people, but that they make decisions without feedback loops, and without a scoreboard, indifference and ignorance look identical from the clinician's perspective. This is part one of a two-part series; next week he'll explore what the scoreboard could actually look like and what happens when you measure the human cost of administrative decisions. 78: I’ll Take My Burnout To Go, Extra Cheese

In this episode, Dr. Junaid unpacks more reasons why physicians are leaving medicine even as traditional burnout rates improve. Drawing on 2024 research from the AMA, Mayo Clinic, and Stanford published in JAMA Network Open, he walks through the data and examines why wellness programs often backfire when offered instead of structural change, and offers one actionable step any physician can take right now. He also shares the Physician Support Line (1-888-409-0141), a free, confidential, non-reportable resource staffed by volunteer psychiatrists who understand your work. Rest refills gas tanks; it cannot repair cracked engines, and this episode is about naming the crack so you stop blaming yourself for it.
