Prednisolone Once Daily vs Thrice-Daily Hydrocortisone in Adrenal Insufficiency: RCT Evidence

Does once-daily low-dose prednisolone change short-term bone and metabolic outcomes compared with thrice-daily hydrocortisone in adults with established adrenal insufficiency? This four-minute Hormone Insight briefing examines a 9-month, double-blind, 2-period crossover randomised clinical trial. Forty-seven adults were randomised and 46 analysed; each regimen was given for 120 days with at least a 2-week washout. Primary findings with prednisolone were lower carboxylated osteocalcin by 1.22 ng/mL (95% CI -2.35 to -0.10; P=.04) and lower undercarboxylated osteocalcin by 1.38 ng/mL (95% CI -2.32 to -0.44; P=.005). Weight, waist circumference, BMI and HbA1c were also lower. Quality of life, lipids, blood pressure and overall adverse-event frequency did not differ significantly. Safety remains important: 133 adverse events occurred in 37 participants, including 3 serious events; 1 participant withdrew for hyponatraemia, there were 17 stress-related dose-doubling episodes, and no adrenal crises. These are short-term surrogate findings, not proof of fewer fractures, cardiovascular events or deaths. Limitations include the small single-centre sample, selected adults already established on replacement, 120-day treatment periods and unadjusted secondary P values. Educational content for doctors and health professionals; not individual treatment advice. Do not change glucocorticoid replacement without specialist review. Narration is AI-generated; no independent human clinical approval is claimed. VESALIUS-CV: Evolocumab Before a First Heart Attack or Stroke

Hormone Insight examines whether evolocumab reduces first major cardiovascular events in a selected high-risk population without previous myocardial infarction or stroke. This is an appraisal of the primary VESALIUS-CV randomised trial, not a new study announcement. We cover the two primary composites, absolute benefit, adverse events, the statistical boundary around mortality findings, and clinical applicability. Key findings: • 12,257 participants; median follow-up 4.6 years. • Coronary heart disease death, myocardial infarction or ischaemic stroke: five-year Kaplan–Meier estimates 6.2% versus 8.0%; hazard ratio 0.75 (95% CI 0.65–0.86). • The 1.8 percentage-point difference corresponds to approximately 18 fewer people per 1,000 experiencing the composite over five years, derived from rounded risk estimates. • Adding ischaemia-driven arterial revascularisation: 13.4% versus 16.2%; hazard ratio 0.81 (95% CI 0.73–0.89). • Serious adverse events: 28.2% versus 28.9%; adverse-event treatment discontinuation: 3.3% versus 3.7%. Safety collection did not comprehensively capture minor adverse effects. Around two thirds already had qualifying atherosclerosis. These results should not be applied indiscriminately to generally healthy adults or everyone with diabetes. Mortality analyses below the failed coronary-death test in the hierarchy remain exploratory. Funded by Amgen; 93% of participants were White.
