Illinois Medicaid and HR1: Common Questions Answered (Extended Edition)

Illinois is preparing to implement major federal Medicaid changes beginning in 2027. The rules themselves are complicated, but the larger challenge is operational: the State must verify income, qualifying activities and exemptions for a large number of Medicaid customers while conducting eligibility redeterminations every six months. In this extended edition of the HR1 Common Questions episode, we take a deeper look at how Illinois plans to implement HR1 and what those plans could mean for Medicaid customers, healthcare providers, advocates and state policymakers. The discussion examines: • Which Medicaid customers fall within the ACA Adult eligibility group• Why most parents and caretaker relatives are treated differently under Illinois FamilyCare• How the $580 monthly MAGI threshold relates to the 80-hour requirement• How income from other members of a Medicaid eligibility household may affect compliance• The interaction between Medicaid and SNAP work rules• Why an adult with a recognized disability should carefully evaluate the financial consequences before requesting a move from ACA Adult coverage to AABD Medicaid• Illinois’s three-tier approach to medical-frailty exemptions• How claims data, healthcare-utilization records and customer attestations may be used• The reduction in federally funded retroactive Medicaid coverage and Illinois’s decision to support an additional month with state funds• The distinction between Form A and Form B renewals• The administrative and technology challenges facing HFS as implementation approaches The episode draws primarily from HFS’s September 15, 2026 “Common Questions and Answers” webinar (https://hfs.illinois.gov/info/fedresctr.html), together with information HFS provided during the spring legislative budget process. For Medicaid customers, the immediate message remains straightforward: make sure HFS has your current mailing address, telephone number and email address; establish access to ABE Manage My Case; and do not ignore renewal forms or other State notices. This episode provides a policy and operational overview based on information available at the time of recording. It is not an individual Medicaid eligibility determination or legal advice. Illinois Medicaid and HR1: Common Questions Answered

Beginning in 2027, some Illinois adults enrolled through the Affordable Care Act expansion group will face new federal Medicaid work and community-engagement requirements, along with eligibility redeterminations every six months. In this focused episode, we examine the most common questions addressed by the Illinois Department of Healthcare and Family Services (HFS) during its September 15th webinar. The discussion covers: • Who may be subject to the new requirements• When $580 in monthly MAGI income can satisfy the requirement without separately documenting 80 hours• How Medicaid and SNAP rules interact• The difference between Form A and Form B redeterminations• How Illinois plans to identify medical-frailty exemptions• When customers may use self-attestation during 2027• Why customers who meet an exemption must still complete Medicaid renewals The episode also explains the immediate steps Medicaid customers can take, including updating their address, telephone number and email address, creating or checking an ABE Manage My Case account, and carefully reviewing every notice received from the State. This episode is based primarily on information provided directly by HFS during its September 15, 2026 webinar (https://hfs.illinois.gov/info/fedresctr.html). It is intended as a general educational overview and not as an individual Medicaid eligibility determination or legal advice. This is a public episode.
