The Exclusion Cycle: Why AI Insurance Repeats Cyber

AI is entering insurance the same way cyber did a decade ago. It gets written into policies quietly, with no real data behind it, until the claims start coming and the carriers react by excluding the risk. That exclusion opens a gap, and MGAs and specialists move into it faster than the incumbents can price their way back. That prediction comes from Susan Holliday, our first guest, who has watched this exact pattern play out twice from the capital side of the trade. In this episode she explains why the exclusion cycle repeats by design, why an abundance of historical data can be a false sense of security when the data is no longer relevant, and where the adoption of AI actually breaks: not at the board or the C-suite, both of which she describes as eager for it, but in the operational middle, at the point where the work has to be implemented. It is the same place digital transformation failed. Susan also shows the pattern is fixable, not just diagnosed. She was part of restructuring Swiss Re into a holding company with per-division accountability, a change her legal team first called impossible, and she describes what it took: senior conviction and a genuine mindset change, not a new org chart alone. Susan Holliday is currently a public and private company board member and advisor. An episode where the data meets the risk from both ends of the same pipe. Kevin Henderson has spent 20+ years in telematics and sensor data, including processing billions of miles of telematics data across multiple continents, when he took what was then the world’s largest commercial fleet telematics dataset from zero monetization to revenue. He is a published contributor to Carrier Management with multiple Editor’s Pick recognitions. ©️2026 Indenseo Corporation Get full access to Structural Signal at structuralsignal.com/subscribe

When the Operating Layer Breaks

Episode 2: When the Operating Layer Breaks 138 vessels crossed the Strait of Hormuz every day before the February strikes. By March 9, one ship crossed. The waterway was physically open. A cancellation notice from an insurance company in Norway stopped 20 percent of the world's oil supply. This episode traces the cascade from maritime war risk through LNG markets, fertilizer costs, and semiconductor supply chains, to 200 stranded helium containers with a 45-day boiloff clock and $650 billion in AI infrastructure at risk. It also covers California (668,000 homes on the state's insurer of last resort, up from 140,000 seven years ago), Australia (home premiums up 178 percent in a decade), and Florida (the market that demonstrates what recovery looks like when litigation reform removes the noise from the operating layer). The episode ends with the dual failure: supply chain disruption from the Hormuz insurance withdrawal on one side, and the simultaneous withdrawal of AI-related coverage by U.S. carriers on the other. The operating layer is failing at both ends of the same economic activity at the same time. The Operating Layer

Eighty-two percent of insurance carriers have a telematics program. Forty percent actually use the data. The technology works. The data exists. The carriers bought it, deployed it, and announced it on stage at conferences. So why is more than half the industry not using what they paid for? In this first episode, I lay out the framework for everything this show will cover. I call it the operating layer: the space between having data and getting value from it. The messy middle where models, technology, and capital meet actual organizations run by actual humans. I walk through the Spectrum (theorists on one end, evangelists on the other, and why both are right but both fail), the Telematics Adoption Paradox, why the people who resist your technology are usually being perfectly rational, and what the companies that survived the operating layer did differently. The patterns are drawn from insurance and telematics because that is where I have spent 20+ years. But the thesis is universal. If you are sitting on sensor data in any industry and you cannot figure out why the gold mine has not produced gold, this episode is about your problem too. This episode includes a companion diagnostic scorecard: 10 indicators, five on the theorist side and five on the evangelist side. Score yourself 0 to 2 on each. Your totals tell you whether you are in the Theorist Trap, the Evangelist Trap, or the Messy Middle. The scorecard is a free PDF download inside the companion analysis below. Structural Signal - Trailer

Eighty percent of companies that try to monetize their data fail. Not because the data is bad. Not because the technology doesn't work. They fail in the middle, in the space between having data and getting value from it. Structural Signal is about that middle. Host Kevin Henderson calls it the operating layer: what actually happens when models, technology, and capital meet real organizations run by real humans. Kevin draws on 20+ years of operational experience, from processing billions of miles of telematics data across multiple continents to building data monetization programs from zero to revenue. If you are an insurance executive wondering why your telematics program still isn't delivering results, start here. If you are sitting on sensor data and can't figure out why the gold mine hasn't produced gold, this show is about your problem. If you want to understand the hidden markets being built on telematics and sensor data, we map out how they actually work every week. Published weekly. Produced by Indenseo.
