8,736 Hours: The Self-Management Framework That Prevents Clinician Burnout and Heals Chronic Disease

Personal Opener I was standing in front of 72 healthcare leaders when I said the number 8,736 out loud and watched the room go quiet. That is how many hours every person in that room manages their own health without a healthcare clinician present. This episode shares the full five-pillar self-management framework, and the one piece of confidence science that changes how you set goals, how you coach patients, and how you show up for your team.  Episode Summary What if the single most powerful tool for healing healthcare is not a new technology, a policy change, or a wellness benefit? What if it is the skill of self-management, and what if it starts with the healer, not the patient? In this episode, Karla Cauldwell shares the five-pillar self-management framework she taught to a group of 72 healthcare leaders. She unpacks the research showing that 75% of chronic disease is preventable through health behaviors, explains why confidence is a stronger predictor of success than willpower or discipline, and walks through the story of a chief medical officer whose 10-minute daily practice shifted the culture of an entire clinic. This framework is for you first. Then it is for every patient and colleague you serve. What You Will Learn •       Why 75% of chronic disease is preventable and what that means for your clinical practice and your own health. •       The five pillars of self-management: self-assessment, choice, intentional action, anticipating risk and building confidence, and accountability. •       How to use the Whole Life Healing Assessment Wheel to identify where to start. •       The confidence check: why a score of 7 or above predicts goal completion, and what to ask when the goal needs to shrink. •       How to coach yourself and others from a vague idea to a SMART action they will actually follow through on. •       How the self-management framework connects to the High Reliability Organization model your healthcare system may already use. •       The real-world story of a chief medical officer who changed clinic culture by changing his own behavior first.   Episode Timestamps 00:00   Welcome 00:13   The two questions driving Karla’s work: why is healthcare so broken, and how do we heal it? 01:01   What self-management is and why it is the skill set that matters most 02:25   The 75% statistic: chronic disease and preventable health behaviors 03:18   Pillar 1: Self-assessment and the Whole Life Healing Assessment Wheel 04:41   Pillar 2: Choice and why motivation requires autonomy 06:47   The 8,736 hours: reframing who is responsible for health 08:23   Pillar 3: Intentional action and SMART goal setting with real examples 10:52   Pillar 4: Anticipating risk and the connection to High Reliability Organizations 13:29   Pillar 5: Confidence as the strongest predictor of success 17:45   The 5% improvement question and why small goals win 18:30   Accountability: building a system that holds 21:00   Real-world examples for patients, staff, and leaders 27:04   Next steps: start with yourself  This Week’s Healing Assignment Pull out your calendar. Pick one area from the Whole Life Healing Assessment Wheel: finances, personal growth, movement, nutrition, relationships, managed stress, spirituality, or rest and sleep. Write one SMART goal for this week. Specific, measurable, action-oriented, realistic, and time-sensitive. Then ask yourself: how confident am I that I will complete this, on a scale of 1 to 10? If your answer is below 7, make the goal smaller until it reaches 7 or above. Set up one form of accountability, whether that is a check-in with a colleague, a phone alarm, or a note somewhere you will actually see it. Win that one small thing this week. Let yourself feel the win. One hundred small wins from now, you will not recognize who you have become. Better Sleep for Healthcare Workers: How to Define It, Improve It, and Coach Patients Through It

Sleep is often the biggest challenge for both patients and healthcare staff. In this episode, I walk you through exactly what good sleep and rest look like, what the research says about why it matters for your long-term health, and a five-step framework for building a plan that fits your real life, one you can use on yourself and bring straight to the patients and staff you serve. EPISODE SUMMARY In this episode, Karla Cauldwell breaks down everything healthcare workers need to know about sleep and rest, from what good sleep actually looks like and the science behind why it matters for disease prevention, to a practical menu of interventions and a five-step self-management framework for building a personalized sleep plan. Karla opens with the healthcare worker, because healing starts there, and then walks through how to apply the same coaching framework with the patients and staff you serve. WHAT YOU WILL LEARN How to define good sleep and rest for your unique life and schedule The clinical evidence connecting sleep deprivation to chronic disease, including heart disease, diabetes, obesity, and cognitive decline A practical, personalized list of interventions to investigate based on what is actually keeping you awake The five-step Whole Life Healing Framework for building a sleep improvement plan that is small, realistic, and sustainable How to apply the same five steps when coaching your patients and staff toward better sleep EPISODE TIMESTAMPS 00:00 — Welcome and what we are covering today 01:15 — Defining good sleep: what the research says and how to know it is working for you 03:52 — What rest means beyond sleep, including micro-recovery during your day 04:41 — Why sleep matters: the clinical evidence and disease prevention 07:20 — Why the "perfect sleep" goal works against you and what to do instead 08:18 — Practical interventions: environment, electronics, digestion, stress, caffeine, alcohol 13:43 — Introducing the five-step self-management framework 14:43 — Step 1: Identify what matters most: your North Star 15:35 — Step 2: Self-assess; you are the most important data source 16:36 — Step 3: Pick one action to move your number up by one point 17:13 — Step 4: Make it SMART and run the 75 percent confidence test 21:03 — Step 5: Connect the action back to what matters most 22:19 — Applying the same framework with patients and staff 23:55 — Why health behavior change is different from clinical intervention 25:45 — Closing encouragement and your healing assignment THIS WEEK'S HEALING ASSIGNMENT Rate your sleep on a scale of 1 to 10. Then ask yourself: what is one realistic, specific action I can take this week to move that number up by one point? Not by five. By one. Write it down. Make it SMART: specific, measurable, action-oriented, realistic, and time-sensitive. Run the 75 percent confidence test if you are not at least 75 percent sure you can complete it, rewrite it smaller. Then connect it to what matters most to you and put that connection somewhere you will actually see it. One point. That is your assignment. Stop Outsourcing Your Healing: How Radical Responsibility Is Your Biggest Health Ally

Episode Summary In this episode, Karla Cauldwell shares the single most powerful shift she has witnessed in seventeen years of nursing and health coaching: moving from starting "out there" with experts, books, protocols, and programs to starting "in here," with your own body, your own lived experience, and your own curiosity. The word itself invites a different kind of awareness, one that feels more natural, more alive, and more honest about the way we actually move through our days. Karla walks through how to use the Whole Life Healing Assessment Wheel to assess your rhythms across eight life areas, how to consciously choose and protect the ones that serve you, and how to stay mindful when life changes disrupt the patterns you have worked to build.   Key Insight from This Episode "The goal is to wake up to what your rhythms are and to be cognizant of whether they are supporting the life you want or not."  — Karla Cauldwell   Karla's Values Framework Before assessing rhythms, Karla invites you to get crystal clear on what matters most to you. 7. Move from Overwhelm to Clarity: The Simple Framework for Making Health Changes that Stick

    Apr 12

Move from Overwhelm to Clarity: The Simple Framework for Making Health Changes that Stick

Moving From Overwhelm to Clarity How to Identify One Action Step You Can Confidently Take Have you ever wanted to make a positive change in your health but felt completely overwhelmed by where to start? You're not alone. In this episode of Whole Life Healing, nurse coach Karla Cauldwell shares a simple framework that helps people move from overwhelm to clarity with a defined action step they feel confident completing. This method comes from over 10,000 hours of health coaching experience and is grounded in science-backed behavior change strategies. The key insight? Most people feel overwhelmed because they compare where they are now to a perfect end result. Instead, the path forward becomes clear when we focus on progress, not perfection. What You'll Learn in This Episode ✔ Why overwhelm happens when trying to change health behaviors✔ The simple mindset shift that replaces perfection with progress✔ How to create a baseline using a 1-10 self-assessment✔ The “move up by one” method for identifying your next best step✔ How to turn ideas into action using SMART goals✔ Why connecting your actions to what matters most increases success The Whole Life Healing Framework for Behavior Change Step 1: Identify What You Want to Change and Connect the Change to What Matters Most Choose one area of health you want to improve. Examples include: Nutrition Sleep Stress management Physical activity Energy levels Then ask yourself: Why is this change important to me? Motivation strengthens when we connect our goals to our deeper values. Ask yourself: Why does this matter to me? How will this improve my life? How will it support what matters most? This connection becomes your driver for change. Step 2: Establish Your Baseline Rate yourself on a scale from 1 to 10. 1 = I am just starting10 = I have fully achieved this goal This number gives you clarity and personalization because everyone begins from a different place. Step 3: Move Up By One & Make it S.M.A.R.T. Instead of trying to go from a 3 to a 10, ask: What is one action I can take to move from a 3 to a 4? That single step becomes your focus. Small steps build momentum and confidence. Turn the Idea Into a SMART Action SMART goals help move you from idea to action. SMART means your goal should be: Specific Measurable Action-oriented Realistic Time-sensitive Example: Instead of saying“I need more sleep.” Say: ➡️ “I will go to bed at 10 PM Monday through Friday this week.” Repeat the Process Once the week ends: Reassess your number Identify the next step Move up by one again Over time, your number increases and so does your health. The Key Principle Progress beats perfection every time. When you focus on one realistic step, you create momentum that leads to lasting change. Your Reflection Ask yourself today: 👉 What is one health behavior I want to improve? 👉 Where am I on the scale from 1-10? 👉 What is one action I can take this week to move up by one? Share the Healing When one person improves their health, they influence everyone around them. When we heal ourselves, we role-model healing for our families, communities, and workplaces. Share this episode with someone who may need encouragement today. We are truly better together. I am rooting for your success! Karla This is a public episode.
