    Steroid Stewardship

Welcome to the inaugural episode of Eczema and the Patch Perspective! When a patient walks into your clinic with severe, fissured, bleeding, and recalcitrant hand eczema, what is your knee-jerk reaction? For most of us, it’s reaching for a tube of high-potency topical corticosteroids. But as clinicians, we know the dark side of that choice: the "steroid hamster wheel" of skin atrophy, patient frustration, and predictable rebound flares. In this episode, host Dr. Dathan Hamann, MD, FAAD, introduces a core philosophy of the Contact Dermatitis Institute: Diagnostic and Steroid Stewardship. To put this framework into practice, Dr. Hamann breaks down a groundbreaking new systematic review and meta-analysis recently published in the journal Dermatitis: "Dupilumab for Chronic Hand Eczema" (Riva et al.). We dive deep into the pooled data from 22 studies covering 374 patients to analyze how this IL-4/13 inhibitor is changing the landscape of chronic hand eczema (CHE) management—and exactly why a "biologic-first" approach without patch testing is a major diagnostic trap. The Meta-Analysis At-A-Glance: We look at the impressive headline statistics, including the 80.3% overall response rate of chronic hand eczema to Dupilumab within 4–16 weeks, and what the recent FDA label updates mean for your daily practice. The Th2 "Sweet Spot": Why Dupilumab acts as a clinical rockstar for specific phenotypes like Atopic CHE and Dyshidrotic/Vesicular variants, and how to identify these high-probability responders early. The "Masking" Effect (A Surgeon’s Cautionary Tale): We unpack a critical case report of a surgeon whose widespread eczema cleared on Dupilumab, but whose hand dermatitis completely failed to respond. Dr. Hamann explains why blocking Th2 pathways can "mask" overall inflammation while leaving a Th1-driven Allergic Contact Dermatitis (ACD) completely untouched. The Hyperkeratotic Hurdle & Th-Shifting: Why thick, scaly, hyperkeratotic palms responded the poorest in this systematic review, and the immunologic reality of how blocking Th2 can unmask or trigger a Th1/Th17 psoriasiform shift. The 3 Key Patch Test Perspective Pearls for Your Clinic: Atopy Is Not an Excuse to Skip the Patch: Just because a patient has a history of childhood eczema doesn't mean their current hand flare isn't exogenously triggered. Patch test early to find avoidable allergens so your systemics don't have to do all the heavy lifting. Partial Clearance Is a Diagnostic Clue: If a patient's face and body clear up on Dupilumab but their hands remain angry, you aren't looking at a "treatment failure"—you're looking at a "diagnostic incomplete." It's time to patch test for a hidden contact allergy. Know When to Pivot: When dealing with hyperkeratotic plaques that fail Dupilumab, look past the IL-4/13 receptors. Use patch testing to rule out the allergic component, and prepare to shift your clinical target if a psoriasiform presentation is unmasked. Eczema & The Patch Perspective is a quick podcast brought to you by the Contact Dermatitis Institute. Hosted by Dr. Dathan Hamann, we bridge the gap between cutting-edge dermatology literature and real-world clinical success, providing an expert patch tester's point of view on the hottest topics in allergy and skin disease. Stop guessing. Start testing. If you found these pearls helpful, please leave us a 5-star review on Spotify and share this episode with a colleague fighting the "hand eczema war" in their clinic!
