The premature babies adrenals , Can we reveal the secret ?

Physiological Mechanisms of Cortisol Deficiency Fetal Zone Dominance: During early fetal life, up to 80% of the adrenal cortex consists of the fetal zone, which is primarily programmed to synthesize sex steroids (androgens/estrogens) rather than cortisol[1].3β-HSD Enzyme Deficiency: Extremely preterm adrenals lack adequate expression of 3β-hydroxysteroid dehydrogenase (3β-HSD), the key rate-limiting enzyme required to convert precursors into cortisol[2][4]. Full enzymatic maturation often does not occur until 30 to 33 weeks corrected age[5].Loss of Placental Substrate: In utero, the fetus relies on progesterone supplied by the placenta to synthesize cortisol[3]. Delivery at 24 weeks abruptly cuts off this placental progesterone supply, causing circulating substrate levels to drop precipitously within the first 24 hours of life[6].Blunted HPA Axis Response: In addition to primary adrenal enzymatic constraints, the hypothalamic-pituitary-adrenal (HPA) axis in periviable infants demonstrates a blunted pituitary ACTH and cortisol response during acute postnatal stress[5][7]. 2. Clinical Impacts on Infants Born 24 Weeks Early Refractory Hypotension: Because endogenous cortisol is essential for maintaining vascular tone and catecholamine receptor sensitivity, acute deficiency causes vasopressor-resistant hypotension shortly after birth[8][9]. Registries indicate that more than 50% of infants born at 22–23 weeks require hydrocortisone support during the first three postnatal days to maintain blood pressure[10].Late-Onset Circulatory Collapse: Between 25% and 30% of infants born at 22–23 weeks develop late-onset adrenal insufficiency after the first week of life[11][12]. This manifests as sudden hemodynamic deterioration, refractory hypotension, oliguria, worsening edema, and increased difficulty with fluid management[11][13].Increased Vulnerability to BPD: Cortisol is a primary regulator of lung maturation and inflammatory pathways. In the setting of systemic inflammation, oxygen toxicity, and mechanical ventilation, cortisol deficiency impairs pulmonary recovery and elevates the risk of developing **bronchopulmonary dysplasia (BPD)**[14]. 3. Evidentiary Gaps in Clinical Trials Exclusion from Major RCTs: Landmark randomised controlled trials—including the landmark PREMILOC trial—excluded infants born before 24 weeks' gestation[17][18].Lack of Direct Subgroup Power: Although the physiological rationale for physiological replacement is strongest in the ≤23-week population, existing RCT data remain underpowered or absent for this specific gestational age bracket[18]. Consequently, clinical practice in infants born at 22–23 weeks relies primarily on observational registry studies, physiological rationale, and consensus protocols[19][21].Send us Fan Mail Support the show

The Extended DOPE by Tarek Kotb , when the real life teaches you

#neonatal #newborn #nicugraduate #neonatology #neonatalcare #lungs #ventilator #ventilation do not forget to join the neonatal mechanical ventilation course by Tarek Kotb https://docs.google.com/document/d/1roNmgCYoW4jSDPGp4yHtIKTq3fyYrHxWpf_hQ0JuG2Y/edit?usp=sharing 🎙️🎙️🎙️🎙️🎙️🎙️🎙️🎙️🎙️🎙️🎙️🎙️🎙️ Doctor my baby is desaturating ❓❓❓❓ OK check for DOPE D... displacement of the tube O.. obstruction of the tube P... pneumothorax E.. Equipment failure Can you tell me more ❓❓❓❓ 🛑this is the first thing to do check air entery if more on the right side that means tube displaced in to the rt side so you need to adjust the tag level or may be the tube is too high you can ask what was the tag level note thay there will be chest movt on both sides 🛑if there is obvious secretions suctioning the tube will improve the saturation always remember that while suctioning the tube the pstient will be more hypoxic as when passing the suction catheter you litterally occluding the tube so you should be fast and do not tske more than 5 seconds with every attempt of suctioning 🛑Pneumothorax you may observe diminished air entery on one side or assymetry of the chest (bulging on one side) or if you have fibreoptic light transillumination will be positive  sometimes it will be tricky as blood gas will show metabolic acidosis 🤔 tension pneumothorax will cause obstructive shock and will decrease the preload so be careful and go and check your patient despite tilling the staff give bicarb 😡😡 🛑Equipment failure means that the machine not delivering the pressure which is needed always check the connections carefully. What else please Can you tell me more ❓❓ Ok🎙️🎙️🎙️ 2ndly if you rulled out DOPE - check for the leak and minute ventilation you may find too much leak (review the ETT size) or minute ventilation is too much less that mean something wrong in the settings . look to the compliance and resistance Not only that have a screening look for the ventilator graphics the curves and loops it will definitely may give you a clue  🛑 Shall i do an x ray 😇 ❓ sometimes you need to do an x ray as sometimes difficult to rule out pneumothorax especially in full term babies x ray also may tell you that there is pulmonary hemorrhages a common problem in ELBW neonates esp in the presence of PDA x ray also may tell you that the lung is hyperinflated too much and you can see the heart is compressed by the overinflated lung so you need to decrease the MAP 🛑 Any otherthing you need to add? 🙈🙉🙊 SOME DIFFICULT SITUATIONS Like pulmonary hypertension crises baby will be ok initially but fluctuation in saturation is the initial clue check preductal and postductal saturation be alert and expect the worst esp if there is history of meconium or hypoxia 🛑 We did an x ray as you know, Is blood gas will add something ❓❓ yes, sometimes you need to do blood gas to troubleshoot the problem esp if you need to adjust the settings and know where you are.  NB: other scenario is the baby who is fully awake and fighting the ventilator always check your baby clinically as you may find his color is ok (pink) and CRT is ok here check the pulse oximeter may be not nicely attached or may ne the patient is moving too much ❌❌Things never to do them if the nurse told you baby is desating never tell her increase FIO2 without checking the baby 2nd never increase the vent parameters if the nurse brought to you a bad blood gas which was previously perfect go and check your patient before playing with the parameters This is our NICU life waiting for the podcast by Tarek's Neonatology Send us Fan Mail Support the show
