    Somewhere in a hospital tonight, a person is being asked to count backwards from ten. They will make it, perhaps, to eight. Between eight and seven, something quietly extraordinary will happen. The self they have carried through every waking moment of their life will simply fold away. Not into dream, not into memory, but into an absence that has no inside. And then, sometime later, without any effort of their own, they will return. This is one of the strangest and gentlest tricks the human body permits, and tonight, on SleepWise, we drift very slowly through the story of it. We begin long before hospitals, when a person in pain had only the oldest medicines to reach for — poppy pressed into wine, mandrake root, the “monk’s sponge” of the medieval infirmaries — and we move gently forward through the moment in 1842 when a country doctor in Georgia named Crawford Long first used ether to remove a small tumour from a friend’s neck, and the moment on October 16 1846 when William Morton stood in the Ether Dome at Massachusetts General Hospital and demonstrated to a room full of surgeons that surgery need not be a scream. We pass through Sir James Simpson and his household experiments with chloroform in 1847, Queen Victoria accepting it “à la reine” for the birth of Prince Leopold in 1853, Humphry Davy giggling into a bag of nitrous oxide in a Bristol laboratory, and the slow careful assembly of modern anesthesia as an orchestra of drugs: propofol quieting the GABA-A receptors, sevoflurane drifting in through a mask, muscle relaxants and opioids arriving in their measured turns. And then we turn, softly, to what is actually happening in the brain when the count reaches seven. We consider the thalamocortical disconnect — the way the great communication highway between the deep brain and the cortex is quietly interrupted, so that signals still arrive but no longer weave into experience. We think about integrated information theory, and Giulio Tononi’s suggestion that consciousness may be what an integrated system feels like from the inside, and what it might mean for that integration to briefly stop. We notice the peculiar honesty of anesthesia, which teaches us that “unconscious” is not the same as “asleep,” and that the return, when it comes, is layered — first hearing, then touch, then the small return of the self, folding itself back into place as if it had never left.
