    Can Tongue Stimulation Help Rewire the Brain in MS?

    In this episode of Neuro Simplified, we review a pilot study on people with multiple sclerosis that combined noninvasive tongue stimulation using the Portable Neuromodulation Stimulator, or PoNS, with intensive physical therapy and working-memory training. The study looked at whether this approach could improve balance, gait, working memory, and whether those changes could be seen on fMRI. Fourteen participants with MS were split into active and sham stimulation groups. Both groups completed a 14-week program that included intensive physical therapy, working-memory training, balance testing, gait testing, neuropsychological assessment, and fMRI before and after treatment. The active PoNS group showed significant improvement on the Sensory Organization Test, while fMRI showed changes in the left primary motor cortex. Both groups improved on working-memory tasks, but only the active group showed increased dorsolateral prefrontal cortex activity. The big clinical takeaway: this was a small pilot study, so we cannot overstate the findings. But the results suggest that pairing intensive rehab with neuromodulation may enhance balance, working memory, and brain activation patterns in people with MS. It also gives clinicians a useful reminder that neuroplasticity is not just a concept. With the right combination of repetition, intensity, task-specific training, and sensory input, we may be able to measure changes in the brain.     Bladder Problems After Stroke or Spinal Cord Injury: What Patients and Clinicians Should Know

    Why do bladder problems occur after a stroke or spinal cord injury, and what can be done about them? In this episode of Neuro Simplified, we review neurogenic bladder and neurogenic voiding dysfunction following stroke and spinal cord injury. These neurological injuries can disrupt voluntary bladder control, leading to urinary urgency, incontinence, incomplete emptying, urinary retention, and recurrent urinary tract infections. Without proper treatment and monitoring, complications may include bladder damage, kidney stones, infections, and impaired kidney function. We discuss how the location of the neurological injury affects bladder function. Injuries higher in the spinal cord are more commonly associated with an overactive bladder and poor coordination between the bladder and urinary sphincter, while lower spinal cord or sacral injuries may cause an underactive bladder and urinary retention. After stroke, loss of cortical bladder control frequently contributes to urinary urgency and incontinence.     Can Mobile Apps Improve Stroke Recovery? What the Research Says

    In this episode of Neuro Simplified, we review a systematic review examining how mobile applications may support recovery after stroke. The researchers analyzed 29 studies, including 11 randomized controlled trials, and evaluated therapy apps, rehabilitation videos, educational apps, reminders, and combined digital interventions. The strongest findings supported interactive therapy apps for post-stroke motor recovery and aphasia rehabilitation, along with rehabilitation videos and reminders for improving exercise adherence. However, evidence was mixed or limited for activities of daily living, quality of life, secondary stroke prevention, depression, anxiety, and visual neglect. The review also found that the most effective stroke rehabilitation apps reflected established rehabilitation principles, including repetitive practice, task-specific training, meaningful goals, feedback, multisensory stimulation, rhythmic cueing, and active patient engagement. Technology appears most useful when it reinforces effective therapy rather than simply transferring a worksheet or exercise handout onto a screen.     Can Vibration Therapy Decrease Spasticity After Stroke

    In this episode of Neuro Simplified, we review a systematic review and meta-analysis on the effects of vibration therapy for people with post-stroke spasticity. The study included 12 randomized controlled trials with 442 participants and examined both whole-body vibration and local muscle vibration. The results suggest that vibration therapy may help reduce post-stroke spasticity and pain while also supporting improvements in motor function. However, the evidence did not show a significant improvement in gait performance.
