    002. Rewriting the Genetic Rulebook for Leukemia

    The "genetic rulebook" for Acute Myeloid Leukemia (AML) has undergone a fundamental transformation, shifting the focus of diagnosis and treatment from how cells look under a microscope (morphology) to their underlying molecular and cytogenetic signatures. This evolution is codified in the competing 2022 classification systems from the World Health Organization (WHO) and the International Consensus Classification (ICC), as well as the updated 2022 European LeukemiaNet (ELN) risk stratification. One of the most significant changes in the modern rulebook is the de-emphasis of the traditional 20% blast threshold for diagnosis. WHO 2022: Eliminates the blast threshold entirely for specific entities, such as AML with mutated NPM1 or translocations involving KMT2A, NUP98, and MECOM.ICC 2022: Reduces the requirement to 10% blasts for these defining genetic abnormalities and introduces a new category called MDS/AML for cases with 10% to 19% blasts that harbor specific MDS-associated mutations.Exceptions: Both systems still require a 20% blast count for AML with the BCR::ABL1 fusion to distinguish it from the accelerated phase of Chronic Myeloid Leukemia.The 2022 ELN guidelines have "rewritten" how specific mutations determine a patient’s prognosis: The FLT3-ITD Shift: Previously, the FLT3-ITD allelic ratio was critical for determining risk.
