    Vancomycin Podcast – Pharmacology, Adverse Effects, and Monitoring

    In this episode, we are going to take a closer look at Vancomycin, one of the most widely used antibiotics in the hospital setting and a medication that pharmacists frequently monitor. Vancomycin is a glycopeptide antibiotic primarily used to treat serious gram-positive infections, including those caused by Methicillin-resistant Staphylococcus aureus (MRSA). Its mechanism of action involves inhibiting bacterial cell wall synthesis by binding to the D-alanine–D-alanine portion of peptidoglycan precursors, which ultimately prevents the bacteria from forming a stable cell wall. Clinically, vancomycin is commonly used for infections such as bacteremia, endocarditis, osteomyelitis, and severe skin and soft tissue infections when resistant gram-positive organisms are suspected. One of the most important aspects of vancomycin therapy is therapeutic drug monitoring, as maintaining appropriate exposure is critical for both efficacy and safety. Current practice often focuses on achieving target AUC-to-MIC ratios rather than relying solely on trough levels. Pharmacists also play an important role in adjusting doses based on renal function and monitoring for adverse effects. Two key safety concerns with vancomycin are nephrotoxicity and vancomycin infusion-related reactions such as “red man syndrome,” which is characterized by flushing, rash, and hypotension if the medication is infused too rapidly.
