It has been present for 1 year and is associated with intermittent pruritis. She had seen her primary care physician for this and was prescribed a topical antifungal cream and powder. Over the past few months, she has also experienced a 60 lb weight loss, shortness of breath, and generalized fatigue. She was admitted to the hospital where her lab tests were notable for hypercalcemia (16.6 mg/dL), elevated alkaline phosphatase (305 U/L), aspartate aminotransferase (52 U/L), alanine aminotransferase (24 U/L), and total bilirubin (0.8 mg/dL).
