Ep 170: Stress Less, Live Better

New work shows stress may be associated with cognitive decline. There are many simple ways to manage and even avoid stress Sources: https://www.medpagetoday.com/meetingcoverage/aaic/122252 https://worldhealth.net/news/creative-activities-manage-stress-effectively/ Key Takeaways: - The fight or flight response suits short physical threats, but most modern stress (money, work, relationships, world events) has no physical solution, so it can linger. - A prospective study found higher cumulative stress, reflected in salivary cortisol, was associated with faster cognitive decline. It is an association only, not proof of cause and effect, and stress is not the only factor. - Focus your energy on what you can influence and accept what you cannot. Worrying about things beyond your control just makes the day worse and adds to your stress load. - Regular exercise is one of the best stress tools, partly because it helps discharge the hormones of the fight or flight response. - Simple, low-pressure creative activity (art, music, knitting, colouring in, journaling) gives you a reliable off-ramp from stress. A few minutes a day is enough, and no talent is needed. - Slow, deep breathing, time outdoors and staying present all help. They take practice and will never be perfect, but they are within your control. Ep 169: Children Deserve Better

Off label use of GLP-1 medications is growing in 8 to 11 year olds. There is a better way. Sources: https://www.medpagetoday.com/pediatrics/obesity/122883 Key Takeaways: - Prescriptions of GLP-1 medications for children aged 8 to 11 have risen 310-fold, but off a very low base: about 20,000 children, despite the weight loss formulations only being approved from age 12. - Children are not small adults. They are still growing, and nobody knows what these drugs do at that age, especially over years rather than a six-month trial. - The SSRI experience is a warning: drugs tested in adults later showed a higher risk of suicidality in teenagers, so caution is needed when the risks are unknown. - Nobody is born overweight. What children eat comes down to the adults feeding them, and healthy eating is neither complicated nor as expensive as claimed, with vegetables in season the cheapest food there is. - Reaching for an injection teaches children that the answer to a problem is a medication rather than a change in behaviour, and the better path is going back to basics. Ep 168: Back to Basics

The health benefits of spending time in nature and getting some sunlight are significant. Why don’t we hear more about it? Sources: https://www.medscape.com/viewarticle/nature-prescriptions-when-should-doctors-prescribe-them-2026a1000r62 https://www.medpagetoday.com/neurology/dementia/121893 Key Takeaways: - Nature prescriptions turn familiar advice into a structured approach, but getting outdoors is simple and doesn't need to be complicated. - For most of human history we lived and worked outdoors. Urban life has left many of us disconnected from nature, so we now need to be reminded to get out there. - Time in nature is linked to lower stress and anxiety, and in one study people without their devices showed a significant drop in stress within three days. Research also suggests possible benefits for blood pressure, cardiovascular mortality and diabetes outcomes. - A suggested dose is 20 minutes or more, several times a week, adding up to more than two hours. A city park or garden is perfectly suitable, and even photos or a view from a window may help. - Higher daytime light exposure is linked to a lower risk of dementia, with a 16 percent relative reduction over eight years. That is far smaller in absolute terms, but it costs nothing and a backyard or balcony will do. - You don't need programs or expensive gear. Open the door, go for a walk in comfortable shoes, and you get movement, light and fresh air together. Ep 167: Take the Stairs!

The very simple change from taking the escalator to taking the stairs can reduce your risk of heart disease. Another form of exercise also lower your risk of type two diabetes. Sources: https://www.medicalnewstoday.com/articles/climbing-stairs-linked-39-lower-risk-heart-disease-death-large-study-finds#What-did-the-researchers-set-out-to-investigate https://www.medscape.com/viewarticle/resistance-training-may-lower-risk-t2d-2026a1000spb?form=fpf Key Takeaways: A meta-analysis pooling data from around 450,000 people over 14 years found that those who often climbed stairs had a 39 percent lower risk of dying from heart-related issues, and were 24 percent less likely to die from any cause. Sub-analysis pointed to a sweet spot of roughly six flights, or 60 to 70 steps, per day. It doesn't have to be done all at once. Stair climbing isn't a cure in itself. It's a proxy for getting a bit more movement into the day, and people who take the stairs may also be making other choices that help. Nothing in health is a force field. Not smoking, eating sensibly and exercising all lower your risk, but they never take it to zero, and health messaging often implies otherwise. "Find thirty" can backfire by making exercise feel all or nothing. Fifteen minutes beats zero, ten beats zero, and small amounts still count. Labour-saving devices have stripped roughly 2,000 calories a week of incidental movement out of daily life since the 1960s, which is about a full day's intake. Parking further from the door and taking the stairs puts some of that back. Resistance training was associated with substantially lower risk of type two diabetes, particularly across midlife and combined with adequate aerobic activity. Get some guidance first if you haven't trained in a while. Ep 166: People Will Die

The hysterical reaction to the very sensible idea to reduce tobacco excise in Australia and combat criminal gangs was foreshadowed nine years ago Sources: https://www.youtube.com/watch?v=eXWhbUUE4ko https://thenightly.com.au/politics/liberal-leader-angus-taylor-says-80pc-tobacco-excise-cut-will-smash-australias-illegal-tobacco-trade-c-22816391 Key Takeaways: The federal Coalition and One Nation are proposing to slash tobacco excise, reversing years of steep tax hikes that were meant to cut smoking but haven't worked. Wastewater testing shows nicotine consumption is actually up around 44% since 2017, with roughly 88% of the total nicotine market now illicit. Despite Australia having the highest per stick cigarette prices in the world, tobacco excise revenue has nearly halved, from about $16 billion in 2019/20 to roughly $8 billion this year, with a further drop projected next year. Organised crime gangs are cashing in on the black market, highlighted by the 300th firebombing of a tobacco retailer in recent times. Countries with lower taxation and easier access to vaping, including the UK, Japan, Sweden and New Zealand, have falling smoking rates, and the Cochrane Collaboration has again confirmed vaping is the most effective stop smoking method. Dr Joe argues Australia should adopt the New Zealand model: cut excise to a sensible level and make vapes a consumer product rather than prescription only, shrinking the black market instead of feeding it. Ep 165: Freedom, AI and Doing Your Best Work with Paul Faix

Dr Joe is joined by Paul Faix of Fortix to talk about, his book The Search for Freedom and AI. Paul Faix: https://paulfaix.com/search-for-freedom/ https://fortix.com Key Takeaways: - Growing up in communist Czechoslovakia and watching the regime fall taught Paul to question the "truths" he was handed, making critical thinking the thread running through his life. - The pandemic echoed that experience: those who asked questions and got no answers became a minority that was turned on, a warning about any government claiming a monopoly on truth. - AI is here to stay and will handle the cold, automated work, so the human skills it can't replicate, connecting, empathising, communicating well, become more valuable, not less. - Freedom is different for everyone: it comes from understanding who you are, taking responsibility rather than blaming outside forces, and doing your best work as your best self. - Whatever you're building, finish it. It doesn't need to be perfect, but finishing something is a step toward your own freedom. Ep 163: Rethinking Pain

The standard approach to pain and inflammation is to medicate it. What if there are other factors within our control that could help?
