    How to Speak by Patrick Henry Winston

    I found this excellent post by @cyrilXBT on X (formerly Twitter). It is about how to present your thoughts, how to speak to an audience, how to communicate effectively and how to make an impact. I use all his advice. A MIT professor gave a 1-hour lecture in 2019 that has 18 million views. He died 5 months after recording it. It was his final gift to the world. Patrick Winston taught at MIT for 50 years. The smartest engineers on earth sat in his classroom. And he spent his last lecture teaching them the one skill their degrees never covered. How to speak. 15 lessons that will change how you communicate forever: Never open with a joke. Your audience is not ready to laugh yet. Open with a promise of what they will know by the end. Your ideas are like your children. You are too close to them. What is obvious to you is invisible to everyone else. Explain the obvious. The 5-minute rule: the first 5 minutes of any talk determine whether people will listen for the next 55. Spend more time on your opening than anything else. Repeat your most important idea 3 times in 3 different ways. Once is never enough. Build a fence around your idea. Tell people what it is NOT before you tell them what it IS. Verbal punctuation. Pause. Let the idea land before moving to the next one. Ask questions nobody will answer. Then wait 7 seconds. The silence is not awkward. It is processing. Never read your slides. Your audience can read. They cannot listen and read simultaneously. Use the board not the slides. Writing forces you to slow down. Slowing down forces clarity. Inspire before you inform. Nobody learns from someone they are not inspired by. End with a contribution not a summary. Tell them what you gave them. Not what you said. Never say thank you at the end. It is weak. End with something that lands. Stories make ideas stick. Data makes ideas understood. You need both. In that order. The quality of your communication determines the quality of your ideas in the eyes of the world. Not the ideas themselves. Practice is not preparation. Practice IS the skill. Patrick Winston understood something most people spend their entire careers missing. Your ideas are only as powerful as your ability to transfer them into someone else’s mind. You can be the smartest person in the room and be completely invisible. Or you can master communication and make average ideas feel like breakthroughs.     Magnetic Chairs and Supramaximal Muscle Contractions

    Are we trusting science, or marketing? At the ISCG 2026 congress, I presented a critical analysis of one of the most promoted technologies in pelvic health today: magnetic field chairs. These devices claim to induce supramaximal pelvic floor muscle contractions and to be more effective than physiotherapy. Strong claims. Widely marketed. Repeated across scientific publications. So I asked a simple question:Where is the objective evidence? After reviewing more than 100 scientific articles, one fact became clear:none of them directly measured pelvic floor muscle contraction during the use of these devices. No objective quantification.No direct validation.Only assumptions. To go further, I conducted my own test. The result was clear: the contraction generated was approximately 10% of my maximum voluntary contraction. This is not a minor detail. It fundamentally challenges the core claim behind these technologies. This issue goes beyond one device or one field. We have seen similar patterns before, in cases such as Theranos, the opioid crisis, and surgical meshes in gynecology. Different contexts, same problem: claims ahead of evidence. Science without methodological rigor is not science.And publications without transparency do not produce reliable results. This presentation is not about rejecting innovation. It is about demanding evidence. Because patient safety cannot be based on assumptions.     Carboxytherapy for Post Op Care for Gynecologic Cancer Surgery

    I have used Carboxytherapy for almost a decade to make sure my patients have the best skin before surgery and to have the best recovery after surgery. For the first time the problem of gynecologic oncology vulvar surgery healing is addressed specifically by Dr. Ramos. Eye opening results.     Clitoroplasties: Available Techniques

    As the use of Testosterone shots/pellets/gels/creams goes up so does the sheer number of the enlarged clitoris complaints also. It can be an aesthetic problem as well as functional dilemma. Dulce presents a marvelous presentation on this relatively recent phenomena and reviews the current available techniques used around the world to treat the problem surgically when conservative management is not enough. Stopping Testosterone alone often does not solve the problem. Pros and Cons, good and bad, and her personal reflections on the various techniques are covered.     ISCG in Pictures: A Look Back

    CAVS (Congress on Aesthetic Vulvovaginal Surgery) and ISCG (International Society of Cosmetogynecology) grew up together for a decade starting about 20 years ago. The goals were to introduce and expand Aesthetic/Cosmetic Gynecology in the United States and Abroad. After CAVS had its last Congress in 2015, the two Brother Societies joined. Now all grown up, ISCG reigns supreme. Congratulations to the Pelosis for the perseverance, foresight, structure, sheer will and determination to help create our field of Cosmetic Gynecology. In fact, the term “Cosmetogynecology” was created by Team Pelosi. To those of you who are landing in Fort Lauderdale, Florida, for our annual ISCG shindig, do enjoy the moments and be amazed at where we are now and what lies ahead.
