    Send us Fan Mail The pelvic floor muscles support organ function through two primary roles: providing physical structural support to keep organs in place, and acting as active constrictors to regulate continence and excretion. 1. Structural Support and Maintaining Organ Position The pelvic floor muscles form a physical "floor" or diaphragm that supports the pelvic organs (viscera) against gravity and pressure. The Pelvic Diaphragm: Primarily composed of the levator ani complex (which includes the puborectalis, pubococcygeus, and iliococcygeus) and the coccygeus muscles, this funnel-shaped muscular sheet stretches across the pelvic cavity. It forms a broad, supportive sling that holds the pelvic viscera in place, including the bladder, urethra, vagina, uterus, prostate, rectum, and anal canal. Resisting Downward Pressure: The levator ani actively maintains the position of these organs and resists downward forces during moments of raised intra-abdominal pressure, such as coughing, straining, or lifting. The Urogenital Diaphragm: Located below the pelvic diaphragm, this fibromuscular plate (comprising the deep transverse perineal muscle and associated fascia) supports the distal urethra and vagina, closing the anterior gap in the levator ani.
