Then a job that was personal: her mother is an independent pediatrician who started the practice when Emily was in high school. Twelve years and hundreds of pediatricians later, she is still in it. Why the work matters Pediatrics is often overlooked and underpaid. Independent practices stay open only if the revenue actually comes in. Altus is not “bill the visit, get paid.” It is: what are your goals, a second location, another provider, and what do we do with the money you were already owed?     Where There’s Gratitude, There’s No Fear Ep 45: Dr. Pradnya Mitroo on Private GI Practice

    ☕ Coffee Playdate Podcast with Dr. Pradnya Mitroo Dr. Pradnya Mitroo is a gastroenterologist in Fresno and President of Fresno Digestive Health. She grew up in New Jersey, trained at Mount Sinai and Thomas Jefferson, and built a career around a simple why: her mom lived with GI illness and never got the care she needed — especially as a woman. She drinks tea, not coffee. Black tea, cardamom, no sugar. The path She almost stayed in academics and clinical-trials research. Politics and a lack of impact pushed her west in 2007. She joined a solo GI (rare on the East Coast), grew a largely female patient panel (about 90% women, plus their husbands and sons), then became an owner. The group later consolidated. Her point: independent practice still exists if you treat it as a business. What she is proud of Quality at a lower price than hospital-based care. Outpatient ASCs can run about a third of hospital procedure cost. Autonomy beats a hospital job — she says she would leave medicine before she would work for a hospital. The hard part they never taught Insurance cuts every year. You have to innovate. Med school taught GI, not how to run a practice. If the practice is not viable, you cannot take care of patients the way you want. Culture is part of that: gratitude journals for staff, first 10 minutes of the day, mini retreats since COVID, reminder that they are not “pushing papers” — they are saving lives. Staff voice = retention. Turnover is expensive. Advice Get a mentor. You will not see every pitfall. Failure teaches more than a win. Book Who Not How — it is the who, not the how.
