Who Supports the Ones Who Show Up?

Remember the neighborhood pharmacist who knew your mother’s prescriptions by heart? Most of us traded that relationship for convenience, and we didn’t notice until it was gone. In this episode, Mavis J. shows how the same story is playing out with our doctors. Private practice has dropped from about six in ten physicians to about four in ten in just over a decade. Close to eight out of ten doctors now work for a hospital system or a corporation, and nearly half are still reporting burnout. Mavis has run a small private practice alongside her husband for fifteen years, and she has seen it from the inside. She introduces what she calls “the one-way bridge.” Hospital systems already know how to build partnerships with small practices; they tend to build the ones where the benefit flows toward them. Using decades of research on reciprocity and on feeling supported at work, she makes the case for a fairer relationship. That could mean access to group health insurance, wellness and burnout programs, small perks, and real help with the paperwork that keeps doctors from their patients. Most of all, she argues for choice, so patients can still pick the small practice that feels like it belongs to the neighborhood. You don’t have to work in healthcare to recognize yourself here. If you’ve ever been the small vendor, the always-on-call contractor, or the department everyone leans on and nobody thanks, you have stood on a one-way bridge too. In this episode * What the disappearing neighborhood pharmacy can teach us about our doctors * How quickly private practice has shrunk, and who doctors work for now * The one-way bridge, and why it isn’t only a healthcare problem * The age-old rule of reciprocity, and what feeling supported does for people at work * Four kinds of support that could help keep small practices alive * Why patients deserve a real choice in the kind of care they get This is a public episode. Who Takes Care of the Caregiver?

Burnout does not announce itself. There is no single event behind it, no baby, no layoff, no aging parent who suddenly needs care. It builds quietly until leaving feels like the only door left. In Episode 3, host Mavis J. looks at burnout as its own kind of opt-out, and she looks at it through healthcare, where the people trained to notice suffering in others are the least likely to notice it in themselves. Mavis walks through the data. More than half of nurses report burnout, and nearly three in four feel emotionally drained several times a week. Many physicians avoid mental health care because they fear what it could do to their license or their career. She also looks at a 2025 JAMA Psychiatry study that found 79 percent of physician suicides were men, even though female physicians face a higher risk than other women. The message of the episode is simple. Burnout is not only a women’s story. It is a human one. Women tend to say the exhaustion out loud, and men tend to go quiet. Both are burnout, and only one gets noticed in time. In this episode * Why burnout is a different kind of opt-out * Burnout and help-seeking rates among nurses and physicians * What a 2025 JAMA Psychiatry study shows about gender and physician suicide * Why burnout is a human occurrence, not a female one * The blind spot in a profession built to spot danger in everyone but itself * The one question to ask the healthcare worker in your life this week If this episode hit close to home, the 988 Suicide and Crisis Lifeline is available any hour, by call or text.
