When a run goes red, the fact of failure tells you almost nothing: it can be a product bug, a broken test, stale data, a dead environment, a contract change, or a known issue that already has a ticket. Our rule is one defect per cause in Allure TestOps, a matcher rule on every defect so recurring failures link themselves in later runs, and a tracker issue created through the integration when the cause is a genuine product bug. On top of that we connected Cursor / Claude Code to the TestOps and tracker MCP servers and wrote a /testops-run-defects skill that walks a run end to end: it pulls unresolved failures, groups them by shared cause, checks existing defects so it doesn't create duplicates, separates product bugs from test, data, environment and Swagger problems, creates defects with matchers, and fills in the tracker fields. On one real run: 29 unresolved failed/broken results processed, 9 defects created, 9 tracker issues linked, 0 unresolved results left.
