Deep dive into an interesting set of issues

I’m not in the mining or quarrying industries and I’m not even an engineer but I’m a massive fan of the Brady Heywood podcast and this spinoff was well worth a listen. I deeply appreciated the exploration of “drift into failure” and high reliability organizations. I found myself backing up to re-listen to parts that were particularly enlightening.
