S. 1 E. 4 - Hearts With A Mission - Transitional Living Program

I had the great opportunity to sit down with Taylor Ann (TLP Case Manager) and Mariah (Community Outreach and Education Coordinator) and learn more about the Transitional Living Program. This program serves both in Josephine and Jackson County. 

We also briefly talked about the Safe Families, a short-term housing program that consists of a host family voluntarily take in children 0-22. Please see attachment for further details.  

PY+ has teamed up with HWAM several times in a variety of capacities to serve youth. It has been an honor to actively serve, challenge and encourage youth together in both Jackson and Josephine Counties.
