(Audio) Prostate MRI PI-RADS 4: Do You Need a Biopsy?

Hearing that your prostate MRI shows a PI-RADS 4 lesion can be frightening. Does it mean you have prostate cancer? Do you need a biopsy? What should happen next? In this episode of UroPatient, host Vic Senese, RN, BSN, FAUNA, explains what a PI-RADS 4 prostate MRI resultmeans in clear, patient-friendly language. With more than 40 years of experience in urology nursing, Vic helps patients understand the MRI findings, what questions to ask their healthcare provider, and what to expect during the next steps in the diagnostic process. You'll learn: What the PI-RADS scoring system meansWhy a PI-RADS 4 lesion is considered suspicious for clinically significant prostate cancerThe difference between PI-RADS 3, PI-RADS 4, and PI-RADS 5Why an MRI does not diagnose cancer by itselfWhen a prostate biopsy is recommendedHow MRI-targeted biopsies improve accuracyWhat happens during a transperineal or transrectal prostate biopsyHow PSA, PSA density, family history, age, and other risk factors influence decision-makingWhat questions you should ask your urologist before a biopsyWhat happens if the biopsy is negativeWhy some PI-RADS 4 lesions turn out to be benign conditions such as inflammation or prostatitisA PI-RADS 4 score means there is a high likelihood of clinically significant prostate cancer, but many men with this finding do not ultimately have aggressive cancer. That's why combining MRI findings with your PSA level, medical history, physical examination, and biopsy results provides the most accurate diagnosis. Whether you've recently had a prostate MRI, are preparing for a biopsy, or are helping a loved one understand their diagnosis, this episode will help you make sense of the results and feel more prepared for your next appointment. (Audio) What Is a NeoBladder? Life After Bladder Removal Surgery Explained

What happens when someone needs their bladder removed? Is it possible to urinate normally again? In this episode of UroPatient, host Vic Senese, RN, BSN, FAUNA, explains what a NeoBladder is, how it is created, and what patients need to know about life after bladder removal surgery. A NeoBladder, sometimes called an orthotopic neobladder, is a surgically created replacement bladder made from a section of the patient's own small intestine. It is most commonly considered for patients who undergo radical cystectomy, a surgery that removes the bladder, often because of muscle-invasive bladder cancer. Instead of living with an external urine collection bag or a urinary diversion, some patients may be candidates for a NeoBladder that allows them to pass urine through their natural urethra. In this episode, you will learn: ✅ What a NeoBladder is and why it is created✅ How surgeons use a segment of intestine to make a new urine reservoir✅ Who may be a candidate for NeoBladder surgery✅ How patients learn to empty and control their new bladder✅ What recovery after NeoBladder surgery may involve✅ Potential challenges including leakage, nighttime urination, and urinary retention✅ The importance of follow-up care and patient education A NeoBladder does not work exactly like a natural bladder. Patients must learn new techniques, including timed voiding, pelvic floor awareness, and sometimes catheterization if they cannot completely empty the reservoir. For many patients, a NeoBladder can provide an opportunity for a more natural way of urinating and improved quality of life after bladder removal surgery. However, the decision depends on many factors, including cancer treatment needs, kidney function, overall health, and patient preference. Join Vic Senese, RN, BSN, FAUNA, host of UroPatient, as he breaks down this complex urologic procedure in a way patients and families can understand. UroPatient answers the 4 H questions: How is it done? How much will it hurt? How long does it take? How long is recovery?Whether you or a loved one is facing bladder cancer surgery, urinary diversion, or simply want to understand advanced urologic treatments, this episode provides valuable education.
