The Breast Podcast Ever

Hi, we're Dr. Tiwari and Dr. Kocak--a couple of board certified plastic surgeons at Midwest Breast and Aesthetics located in Columbus, OH. We see patients from all over the country and are dedicated to natural breast surgery options. On this podcast we will discuss all types of surgeries relating to the breast including our specialty breast reconstruction. We do all kinds of advanced breast reconstructions that use natural tissues from other parts of the body like the DIEP flap, SGAP Flap and other more unheard of procedures like the PAP flap, TUG flap and DUG flap which are all natural options used for breast reconstruction. Our team of microsurgeons is experienced in all types of reconstructive and cosmetic procedures such as breast enhancement with fat, breast lifts, breast reduction and we'll even talk about implant removal and capsulectomy. We're really excited to have guests to join us such as former patients to talk about their experiences as well as other guests to chat about their expertise as they relate to the types of things we do. We'd love to hear from YOU! Why Two Surgeons in One Operation Can Mean Better Results

Dr. Saraswat and Dr. Tiwari discuss how Midwest Breast uses a co-surgeon model to improve operating room efficiency without sacrificing outcomes. The conversation focuses on why shorter, more coordinated operations can reduce complication risk and how this approach applies to DIEP flap breast reconstruction and massive weight loss body contouring. Key topics Why operating room efficiency matters for patient outcomes, especially during long reconstructive and contouring procedures. How the co-surgeon model allows two attending surgeons to work in parallel on different parts of an operation. How one surgeon may work on the abdomen while the other works on the chest during immediate breast reconstruction. Why the goal is not simply to move faster, but to reduce total time under anesthesia through better coordination. How longer operations may increase the risk of complications such as infection and blood clots. How the same dual-surgeon approach can be applied to massive weight loss body contouring. Examples of treating separate anatomical areas simultaneously, including the arms and legs or the breasts and abdomen. The important role of physician assistants and the broader operating room team. Why a collaborative, ergonomic workflow can be more effective than the traditional solo-surgeon model. How the dual-surgeon model has helped reduce operating times in complex reconstruction cases. Why GLP-1 Weight Loss Is Creating a New Plastic Surgery Problem

This episode covers how massive weight loss has changed from the era of bariatric surgery alone to the current landscape of GLP-1 driven medical weight loss. Dr. Tiwari and Dr. Saraswat walk through the full range of body contouring options from head to toe, explaining what each procedure fixes and why some operations are more functional while others are more aesthetic.   Key topics In this episode: the shift from traditional bariatric surgery to GLP-1 medications like semaglutide and tirzepatide, and how that has changed the type of patients seeking contouring surgery. Dr. Saraswat explains that GLP-1s can be used not only for weight loss, but also to help patients reach a safer BMI before surgery. The discussion covers the main problems after large weight loss: excess skin, loss of volume, rashes, irritation, and premature aging in areas like the face. The episode breaks down the difference between simply tightening skin and actually repositioning the deeper facial framework.   Connect with Midwest Breast: Website Instagram   Timestamps 00:05 - Introduction to post-bariatric body contouring and the new weight loss landscape 00:31 - How massive weight loss patients are changing in the GLP-1 era 01:14 - Why gastric sleeve patients may still struggle with caloric intake 01:59 - Dr. Saraswat's general surgery background and why it matters 02:17 - The long search for a "magic" weight loss solution 02:47 - GLP-1 medications as a major shift in treatment and surgical prep 03:38 - Using medical weight loss to prepare patients for reconstruction 04:11 - BMI cutoffs and why surgical readiness matters 04:45 - Why many current body contouring patients are medical weight loss patients 05:14 - The two biggest issues after weight loss: excess skin and volume loss 05:41 - Why abdominal skin folds cause rashes, irritation, and infections 06:10 - Going head to toe through post-weight-loss procedures 06:28 - Fat grafting, fillers, face lifts, and neck lifts for the face 07:03 - What a deep plane facelift changes compared with skin tightening alone 08:36 - Direct neck lift and combining neck and facelift procedures 09:25 - The role of the platysma in neck contouring and banding 10:14 - Breast changes after major weight loss in women 11:12 - Why a breast lift may not restore lost volume by itself 11:42 - Auto-augmentation techniques for fuller, more shaped breasts 12:30 - Wise pattern and anchor-type incisions in massive weight loss breast surgery 12:55 - Male chest contouring and when free nipple grafting is used 13:45 - What a panniculectomy removes and why insurance often covers it 14:51 - Full abdominoplasty, belly button relocation, and waist shaping 15:53 - When a fleur-de-lis abdominoplasty is needed for horizontal excess 16:42 - Why the T-junction in fleur-de-lis cases can heal well 17:31 - Belt lipectomy and circumferential contouring around the trunk 17:58 - Buttock lift possibilities during lower body contouring 18:12 - Thighplasty for medial thigh excess and skin irritation 19:11 - Seminar announcement: body contouring, surgical and nonsurgical weight loss, and what patients can expect
