The donor site is commonly taken from areas such as the thigh or buttocks, where skin is relatively abundant. STSGs are generally thinner and more flexible than full-thickness skin grafts, making them easier to cover larger or more irregular surfaces. However, they are more prone to contraction and may have a less desirable cosmetic outcome than full-thickness grafts. In oral and maxillofacial surgery, STSGs are used for a variety of reconstructive purposes, particularly in cases of large soft tissue defects following the resection of oral cancers. After the excision of malignancies like squamous cell carcinoma, which often results in the loss of significant amounts of tissue, an STSG can be applied to cover exposed bone, muscle, or other structures. This helps to protect the underlying tissue, reduce the risk of infection, and promote healing by facilitating revascularization. A key application is covering defects in the oral mucosa and oropharyngeal regions post-tumor resection. STSGs offer several advantages, including their ability to cover extensive areas with relatively minimal donor site morbidity. This is particularly beneficial when large intraoral resections create significant defects that cannot be closed primarily or reconstructed with local flaps. While STSGs may not provide the same level of esthetic or functional restoration as other grafting techniques like free flaps, they are often employed in patients where simpler and quicker options are needed due to the patient’s general health or other constraints. For instance, STSGs are sometimes used as an interim solution in cases where a more definitive reconstructive option, like a microvascular free flap, might be delayed or contraindicated. In summary, STSGs are versatile grafts that serve an important role in the reconstruction of large defects in oral and maxillofacial surgery, especially following cancer resection.     📝 “What are the Key Structures & Boundaries of the Modified Neck Dissection?”

Each level contains different groups of lymph nodes, and understanding their anatomical boundaries helps guide the dissection. Level I (Submental and Submandibular Triangles): Boundaries: • Superior: Mandible • Inferior: Hyoid bone • Medial: Midline of the neck • Lateral: Anterior belly of the digastric muscle (submental triangle) and posterior belly of the digastric muscle (submandibular triangle). Key Structures: • Submandibular gland, marginal mandibular branch of the facial nerve, lingual nerve, hypoglossal nerve (CN XII). Level II (Upper Jugular Nodes): Boundaries: • Superior: Base of the skull • Inferior: Hyoid bone • Anterior: Stylohyoid muscle • Posterior: Posterior border of the sternocleidomastoid muscle Key Structures: • Spinal accessory nerve (cranial nerve XI), internal jugular vein, hypoglossal nerve (CN XII). Level III (Mid-Jugular Nodes): Boundaries: • Superior: Hyoid bone • Inferior: Cricoid cartilage • Anterior: Sternohyoid muscle • Posterior: Posterior border of the sternocleidomastoid muscle Key Structures: • Internal jugular vein, sternocleidomastoid muscle, cervical sensory nerves (cervical plexus). Level IV (Lower Jugular Nodes): Boundaries: • Superior: Cricoid cartilage • Inferior: Clavicle • Anterior: Sternohyoid muscle • Posterior: Posterior border of the sternocleidomastoid muscle Key Structures: • Internal jugular vein, phrenic nerve, brachial plexus, and thoracic duct (on the left). Level V (Posterior Triangle): Boundaries: • Superior: Base of the skull • Inferior: Clavicle • Anterior: Posterior border of the sternocleidomastoid muscle • Posterior: Anterior border of the trapezius muscle Key Structures: • Spinal accessory nerve (cranial nerve XI), cervical plexus, subclavian artery, and vein.
