Episode 271: Why Is My Body Changing in Perimenopause? What It Means for Your Heart, Blood Sugar, and Weight

Many women in their 40s and 50s eat the same way and move the same way they always have. Their bodies change anyway. The waistband gets tighter while the scale barely moves. The labs come back normal. The advice is still to eat less and move more. Dr. Carolyn Moyers talks with Dr. Traci DeSplinter, a board-certified OB/GYN and obesity medicine physician, Menopause Society Certified Practitioner, and founder of Refined Women’s Health, a direct care practice for midlife women in the Houston area. Dr. DeSplinter has more than 25 years in practice and additional training through the National Lipid Association in the management of cholesterol disorders. They talk about how body composition shifts across the menopause transition, why insulin resistance can be present years before a diabetes diagnosis, and which tests are worth a conversation with your doctor. They explain why perimenopause is a window for heart and metabolic health, where hormone therapy fits, and how GLP-1 medications are being used in midlife women. Dr. DeSplinter also takes on the misconceptions that keep women from getting care. This episode is for education and is not a substitute for individual medical care. Please talk with your doctor before starting, stopping, or changing any medication. 🔗 Connect with Dr. Carolyn Moyers 📸 Instagram: @drcarolynmoyers 🎥 YouTube: @drcarolynmoyers 🌐 Website: www.skywomenshealth.com Course: https://courses.drcarolynmoyers.com 🔗 Connect with Dr. Desplinter 📸 Instagram: @tracidesplintermd 🌐 Website: https://www.refinedwomenshealth.com

Episode 265: DIY Hormone Therapy: When “Personalized” Goes Too Far

Hormone therapy can be life-changing. Personalized care does not mean anything goes. In this episode, Dr. Carolyn Moyers explains why recreating a monthly bleed after menopause is not the same thing as restoring a menstrual cycle, why endometrial protection matters when systemic estrogen is used in a woman with a uterus, and what clinicians can consider when oral progesterone is not tolerated. She also takes a nuanced look at off-label vaginal use of 100- or 200-mg micronized progesterone capsules, what the evidence can and cannot tell us about endometrial protection, and why estrogen dose, progesterone dose, duration, bleeding, and follow-up all matter. One important distinction: none of this applies to recommended low-dose vaginal estrogen used for genitourinary syndrome of menopause. That is a different clinical situation, and it is generally not a reason to add a progestogen. This is not an anti-hormone episode. Dr. Moyers prescribes hormone therapy every day. It is an episode about what separates personalized medicine from experimentation. Related listening: Episode 253 — "The Hormone Question I Answer Multiple Times a Day: Progesterone vs Progestins." Important: This episode is educational and is not individualized medical advice. Do not start, stop, or change prescription hormone therapy based on a podcast. Postmenopausal bleeding should be discussed with a clinician and appropriately evaluated. 🔗 Connect with Dr. Carolyn Moyers 📸 Instagram: @drcarolynmoyers 🎥 YouTube: @drcarolynmoyers 🌐 Website: www.skywomenshealth.com World Menopause Day Event - get your tickets https://event.skywomenshealth.com/world-menopause-day-8703 Now is the time to have your voices heard! Episode 264: Menopause in the News: The Truth Behind the Hormones-and-Your Brain Headlines, Plus What Testosterone Really Does

The menopause headlines keep contradicting each other — hormones and Alzheimer’s, testosterone as a miracle fix, and a week later, “none of it holds up.” In this episode, Dr. Carolyn Moyers reads the actual studies behind this month’s headlines and tells you what’s real, what got oversold, and what it means for you. In this episode: • Two new studies on hormones and dementia — and the part the headlines skipped: the benefit didn’t clearly apply to women in typical, natural menopause • Why the benefit concentrated in women who lost estrogen early — and how that lines up with what guidelines already recommend • Risk reduction vs. prevention: why the difference is the whole point • Testosterone for women: what the newest review shows it truly helps — and why it’s not a brain or energy drug • Why there’s still no FDA-approved testosterone product for women — what it means to microdose the men’s version, and what an approved product would change • The FDA public workshop on testosterone in menopausal women (Sept 17, 2026) — and how to add your voice to the official record • A question you can use on the next headline you see This episode is for education and is not medical advice. Talk with your own doctor about what’s right for you. • The FDA workshop “Testosterone Use in Menopausal Women” is open to the public: register to watch, and submit a public comment through docket FDA-2026-N-5479 (comments open through Oct 19, 2026).
