    "All Questions answered about Menopausal Hormone Therapy" with Dr. Amanda Clark

    This incredibly comprehensive presentation given by Dr. Amanda Clark explores the menopause transition and its profound impact on women’s bone health and overall well-being. Drawing on landmark research, including the SWAN study and Women’s Health Initiative, Dr. Clark explains what happens hormonally during perimenopause and menopause and why this is a critical period for bone loss. She reviews the different forms, doses, and regimens of estrogen and progesterone and how women and their clinicians can approach individualized hormone therapy decisions. The presentation examines the benefits and potential risks of hormone therapy, including its effects on breast cancer, cardiovascular health, diabetes, cholesterol, dementia, and bone mineral density. Particular attention is given to preventing and managing menopause-related bone loss and developing long-term strategies to preserve bone strength. She also discusses vaginal estrogen and its role in treating genitourinary symptoms, providing women with an evidence-based framework for making informed decisions about their health. A highly informative Q&A will follow the presentation. Dr. Clark is Professor Emerita at Oregon Health & Science University (OHSU) and a Senior Affiliate Investigator at the Kaiser Permanente Northwest (KPNW) Center for Health Research. She completed an OBGYN residency at OHSU, followed by a fellowship in urogynecology in London, England. She completed a Master's Degree in Clinical Research (MCR) at OHSU. She is board-certified in Obstetrics and Gynecology and in Urogynecology and Reconstructive Pelvic Surgery. She is also a Certified Practitioner of The Menopause Society. She held a full-time faculty appointment at OHSU for 23 years followed by 10 years in practice at Kaiser Permanente. In addition to clinical practice, she has been academically active in both teaching and research at both institutions. She has over 40 years of experience teaching students, residents and fellows, and has broad experience in curriculum development and assessment. Her research has generated numerous publications and she does peer review for several journals and is on the editorial board of the Menopause Society. Currently she serves as faculty in an OHSU ECHO program, Menopause in Primary Care, supported by the Oregon Rural Practice based Research Network and the OHSU Center for Women's Health.

    "The Future Role of Bone Health Medications" with Dr. Mary Bouxsein, Prof Harvard Medical School

    OsteoBoston is honored to host Dr. Mary Bouxsein, a renowned osteoporosis expert and the lead researcher of the groundbreaking SABRE Study. In this talk, Dr. Bouxsein clarifies how the study has revolutionized bone medication development by changing its core definition. Instead of undergoing lengthy trials to prove they prevent fractures, new medications can now be approved based on improved bone mineral density (BMD). By establishing BMD as the FDA’s first surrogate endpoint for osteoporosis, this historic shift has dramatically accelerated drug development, unlocked new funding, and already triggered a wave of new lab openings and FDA submissions. Dr. Bouxsein is the Maurice E. Mueller Professor of Orthopedic Surgery at Harvard Medical School. She earned her PhD in Mechanical Engineering from Stanford University, completed her postdoctoral training at Harvard Medical School, and is only the third woman to attain the rank of Full Professor in Orthopedic Surgery at Harvard. An internationally recognized leader in bone and musculoskeletal research, Dr. Bouxsein has dedicated her career to advancing our understanding of bone strength, skeletal fragility, and fracture prevention. Her research spans osteoporosis, aging, spaceflight, and military training, and she has been continuously funded by NIH, NASA, and the Department of Defense for more than 30 years. Dr. Bouxsein co-directs Harvard's Center for Musculoskeletal Research, serves as President of the American Society for Bone and Mineral Research, is Associate Editor of the Journal of Bone and Mineral Research, serves on the board of the International Osteoporosis Foundation, is a member of the Advisory Council for the National Institute of Arthritis and Musculoskeletal and Skin Diseases at the NIH and as a global leading researcher, Dr. Bouxsein has authored more than 435 peer-reviewed publications. Today, we are especially honored to hear about the landmark SABRE study—Study to Advance BMD (bone mineral density) as a Regulatory Endpoint—which Dr. Bouxsein co-leads. This groundbreaking effort helped gain FDA acceptance of bone mineral density as a surrogate endpoint in osteoporosis clinical trials, transforming the way new osteoporosis treatments can be developed and evaluated. Dr. Bouxsein's work is helping to shape the future of bone health research and treatment, with the potential to benefit millions of people affected by osteoporosis and bone loss.

Dr. Neustadt is on the Corporate Advisory Roundtable (CAR) of the Bone Health and Osteoporosis Foundation (BHOF), Vice President of his state medical association and a frequent speaker at medical conferences. A summary of this program: This talk by Dr. John Neustadt focuses on a more integrative and “whole-person” approach to osteoporosis and fracture prevention. Looking beyond bone density scores alone and focusing on actual fracture risk and bone quality. The idea that many fractures occur in people whose DEXA scans are not in the osteoporosis range. Discussion of lifestyle and medical factors that weaken bone, including poor sleep, inflammation, acid-blocking medications, SSRIs, inactivity, nutrient deficiencies, and hormonal changes. Emphasis on nutrition — especially adequate protein intake, Mediterranean-style eating, vitamin D, calcium, and vitamin K2 (particularly MK-4). A review of exercise, muscle strength, balance, and fall prevention as critical parts of bone protection. Discussion of hormone health and how estrogen, progesterone, and testosterone can influence bone remodeling. Cautions about relying only on medications without addressing underlying contributors to bone fragility. Encouragement for patients to become educated advocates for their own bone health plans. Dr. Neustadt also emphasized that osteoporosis develops gradually over decades and that improving bone health requires combining medical evaluation with nutrition, movement, sleep, and metabolic health strategies.
