"Kiss My A**, I Paid a Lot for It"

Alix Earle says she paid for it, and Dr. Chang isn't buying it.  Slide 14 of a photo dump from a 25-year-old TikTok star with eight million followers reads "Kiss my ass, I paid a lot for it" and the internet has already decided what she means. Dr. Chang has a different theory: she is teasing every one of them, and the product everyone names, alloClae, never appears in her caption. Dr. Chang and Jackie O'Brien answer an anonymous question from someone who has hidden a BBL from coworkers for three years and fakes being sick to skip the company beach retreat. Dr. Chang says clothing, not the implant or the fat, decides whether anyone notices, and that the generation posting its whole journey has changed how much secrecy costs. He explains what alloClae is (donor fat, stripped of its DNA into "ghost cells"), why it suits thin frames that have no fat to spare, and how the donor-consent question works. Jackie reports on the Alix Earle Netflix show, where one nipple faces left, and Dr. Chang admits he still has not watched it. Blaming Anesthesia: What the Mica Miller Story Leaves Out

A listener says her mom has come out of general anesthesia twice saying she wasn't herself for weeks afterward, and now with a facelift on the calendar, the whole family wants to know whether to worry.  Jackie O'Brien and Dr. Christopher Chang discuss what actually happens to your mind after anesthesia, and why the honest answer is almost never as scary as it sounds. The question lands the same week two real cases dominate the headlines: a Massachusetts judge declares a mistrial in the Lindsay Clancy trial after the jury deadlocks over a postpartum psychosis defense, and Netflix's Death of the Pastor's Wife revisits Mica Miller's death and her husband's claim that a breast augmentation and a bad reaction to anesthesia changed who she was. Jackie and Dr. Chang talk through what post-op cognitive dysfunction really is, why telling your surgical team about a rough wake-up years ago changes your plan for the better, and the conversation they have when something about a companion in the exam room feels off. NPR, With jury deadlocked, judge declares a mistrial in Lindsay Clancy's case   Netflix, Death of the Pastor's Wife HOSTS Jackie O'Brien RN, BSN, CNOR Clinical Director at Cedar Lane Surgery Center With 12 years of OR experience and training at Georgetown University Hospital, Jackie brings expert-level knowledge in plastic, trauma, general, vascular, and ophthalmic surgery. Rest In Peace, Tiny

Dr. Chang says his whole office might not stock that much filler for every person on the schedule combined. Igho "Tiny" Ubiribo got 40 milliliters of hyaluronic acid injected to his penis in a single sitting at an unnamed clinic in Bangkok and died hours later. Jackie brings Dr. Chang the TMZ story: a London coroner rules that Ubiribo, a 43-year-old British-Nigerian influencer, died of a pulmonary embolism after a Thai clinic injected hyaluronic acid and lidocaine into his penis, and a postmortem finds matching material in his lungs. Dr. Chang traces how injected filler can slip into a vein and travel from the groin to the chest, why the g******s carry so much vascular risk, and what a 50 cc syringe looks like next to what a man already has. Jackie flags the six months it took to name a cause of death as one more reason to think twice about surgery tourism. Dr. Chang's closing advice: a clinic saying yes doesn't make it the right procedure for you. Men Really Only Care About Two Things

"This man needs breast implants immediately," Dr. Chang says about a Swedish triathlete who strapped a bra over his chest mid-race to test whether more volume up top actually cuts wind drag. It does, and the physics backs him up — but Tour de France Femmes rules only allow the real thing, surgically placed. Dr. Chang and co-host Jackie run through three stories that landed the same week, all about what men will do, pay, endure, and hide for their bodies. A North Carolina man crowdfunded over $11,000 in under 12 hours for a penile filler procedure after a micropenis diagnosis, and Dr. Chang explains why filler isn't actually the fix he needs. Then there's the decades-old claim that Donald Trump underwent a painful scalp reduction in 1989, detailed in his ex-wife's sworn divorce deposition — Dr. Chang walks through what that surgery involved and how hair restoration has changed since. They also get into why men still show up to consults alone, what they won't say out loud, and the two things Dr. Chang says every man actually cares about. Destination Disaster: Overseas Surgery Gone Wrong

A woman drains her retirement savings for a 60th birthday makeover in Bangkok and wakes up with her breasts gone entirely. Another pays about three grand for six-pack liposuction and ends up with a wrinkled, distorted abdomen the clinic blames on a signed consent form.  Dr. Christopher Chang and Jackie O'Brien discuss a 60 Minutes Australia investigation into Destination Beauty, one of Bangkok's largest medical tourism operators, and why a free hotel and a five-star review can't replace a surgeon who'll actually see you again. The investigation anchors a wider conversation about medical tourism, from viral (and often fake) "after" photos flooding Instagram and TikTok, to a surgeon caught photoshopping Denise Richards' face onto someone else's patient photo after her actual facelift in Beverly Hills. Dr. Chang explains what a "rookie mistake" on a breast lift actually looks like, why the lowest quote is never the full price once follow-up care and flight risk factor in, and what happens when a routine procedure abroad turns into an infection back home. It's the math nobody runs before booking the flight: who answers the phone when something goes wrong. 60 Minutes Australia, Destination Disaster HOSTS Jackie O'Brien RN, BSN, CNOR Clinical Director at Cedar Lane Surgery Center With 12 years of OR experience and training at Georgetown University Hospital, Jackie brings expert-level knowledge in plastic, trauma, general, vascular, and ophthalmic surgery. Wrong Patient, Wrong Surgery

A woman waiting for a routine procedure asks staff if they have the right person. The reply: “You’ll do.” Minutes later, surgeons remove part of her external genitalia instead of the small skin tag she came in for — and she never even meets the surgeon beforehand. Dr. Christopher Chang and Jackie break down the surgical timeout most people never think about — the ID checks, armband scans, and consent verification every OR runs before the first incision — using the news out of Melbourne’s Sunshine Hospital as the jumping-off point, where Debra Buchanan was mistaken for another woman with the same first name. Jackie also tells her own family’s version of the same mistake: her mother was admitted to the ICU after a heart attack and treated under someone else’s name entirely, a mix-up that took weeks to unwind and delayed her cardiology care. They close out answering a listener question from someone about to get a rhinoplasty who’s more nervous about an administrative slip-up than the surgery itself. He Read It on Reddit So It Must Be True

One Reddit comment was all it took — now her husband keeps a bag of ice within reach around the clock, and after a year of trying, she still isn't pregnant. Dr. Chang has news for the biohackers: the s*****m already auto-regulates its own temperature, and no protocol from a stranger's post knows your body better than it does. Jackie and Carey bring the trends — Medicube and salmon sperm skincare, at-home microneedling, and a clinic advertising non-surgical breast enhancement that amounts to injected saline. The news gets bigger from there: Bryan Johnson posts his sperm count and half a million men copy the protocol, the Department of Defense announces testosterone screening for every service member over 30, and the FDA loosens its prescribing limits. She Tried to Check Herself In. They Said No.

Twelve psychiatric medications from a rotating cast of prescribers who never compared notes, and a detailed journal begging someone to read it. When the same woman asked to be admitted to a psych unit, she was sent home, and Dr. Chang flags exactly why that's the strangest part: a voluntary admission is supposed to be the one door that's always open. Jackie and Carey have been glued to the Lindsay Clancy trial — the Massachusetts labor and delivery nurse whose defense says postpartum psychosis, and the system that brushed her off, led to the unthinkable. Between trial recaps, Dr. Chang answers a listener on Lexapro who's worried her surgeon will turn her down for a mommy makeover, revisits his psychiatry rotation at Bellevue, and explains what he's really screening for when someone discloses a psych history at a consult.
