Why Breast Reduction After Weight Loss Is Really a Breast Lift

A 19-year-old with back pain and a woman who lost 100 pounds might hear the same surgery recommendation, but their needs are completely different and so are their surgical procedures. One needs tissue taken away, and the other needs shape, skin removal, and often a little volume put back.Dr. Vincent Gardner explains why one surgery is more like a breast lift, and why waiting until a 2nd surgery for breast implants gives a more accurate size, and what changes when skin has been stretched by 80 to 100 pounds or more. Find out how close to your goal weight you need to be for breast reduction surgery, how much protein it truly takes to heal well, and why relaxed skin sometimes needs a small revision down the road. Read more about breast reduction: https://www.easternshoreplasticsurgery.com/breast/breast-reduction/ Questions answered by this episode: Is a breast reduction the same as a breast lift?Do I need a breast reduction or a lift after losing 100 pounds? Will I need an implant after a breast lift or reduction?Should a breast lift and implant be done at the same time or staged? How stable does my weight need to be before breast surgery after weight loss?Does breast skin quality differ after gastric bypass, GLP-1s, or diet and exercise? What is the fat roll between the breast and the back, and can liposuction fix it?Are breast surgery scars longer after massive weight loss? Can I combine a breast lift with an arm lift or tummy tuck? What happens if the T junction opens up after a breast lift?Alabama the Beautiful is the cosmetic surgery podcast co-hosted by Dr. James Koehler and Dr. Vincent Gardner, surgeons with over 2 decades of expertise in cosmetic surgery and their trusty co-host Kirstin, your best friend, confidante, and the snarky yet loveable "swiss army knife" of Eastern Shore Cosmetic Surgery. The First Signs of Capsular Contracture After Breast Implants

Months or years after surgery, one side can suddenly feel firmer than the other, and most women assume they did something wrong.Dr. James Koehler and Dr. Vincent Gardner walk through what a mild case actually feels like, how it's graded on the Baker scale, and why some cases never get noticed until something changes years later. The leading theory is a "slime layer" of bacteria that settles on the implant and triggers chronic inflammation, which is part of why implants are more often placed under the muscle today.Treatment carries its own risk of recurrence, so Dr. Koehler explains how an acellular dermal matrix like Strattice dramatically improves the odds of it not coming back — plus what rupture, rippling and bottoming out look like, and why the "replace your implants every 10 years" rule isn't quite true.Read more about capsular contracture treatment: easternshoreplasticsurgery.com/breast/breast-implant-revision/capsular-contracture-revision Questions answered by this episode: What does capsular contracture feel like when it's just starting? How much firmness is normal after breast augmentation? What causes capsular contracture? What is the "slime layer" theory of capsular contracture? What are the Baker grades of capsular contracture? When should capsular contracture be treated?Does a capsulectomy fix capsular contracture for good? What is Strattice and how does it help prevent recurrence? How can you tell if a silicone implant has ruptured?Do breast implants really need to be replaced every 10 years?Alabama the Beautiful is the cosmetic surgery podcast co-hosted by Dr. Facelift: What Recovery Really Looks Like, Day by Day

Most people know what a facelift looks like on the outside. Almost nobody talks about the weeks in between — the swelling, the numbness, the moment a worried spouse finally exhales. Dr. James Koehler walks through the real timeline: the 24-hour dressing change, why he skips drains, when stitches come out, and how long bruising actually takes to settle. The most counterintuitive part might be the six-week mark. Many people look camera-ready by then, but the last of the swelling under the chin doesn't fully resolve for three to six months — and comparing a one-week photo to a friend's isn't a fair test of how surgery went. Koehler also covers what almost never comes up before surgery: temporary numbness around the ears, the rare nerve bruising that can affect a smile for a few weeks, and why spouses tend to worry more than the person having the procedure. Body Lift: What Recovery Really Looks Like, Week by Week

Six months. That's about how long it takes for swelling to fully settle after one of cosmetic surgery's biggest transformations — and the first ten days feel entirely different from month six. Dr. Vincent Gardner walks through the day-by-day reality of a full body lift: soreness, three to four surgical drains, and a new non-narcotic pain protocol that's changing how people get through the first week. He and Dr. James Koehler cover the parts nobody warns you about — where swelling pools in unexpected places, when it's actually safe to drive again, and why most people end up grateful for the scar once the excess skin is gone. Dr. Gardner also breaks down the real timeline for seeing your final shape — and why the first glimpse in the mirror is only the beginning. Can You Really Get Rid of a Scar After Surgery?

There's no such thing as erasing a mark on your skin. The best a cosmetic surgeon can do is trade it for a thinner, flatter line placed where a waistband or bathing suit hides it — which is exactly what a tummy tuck does with an old vertical C-section scar. Dr. James Koehler and Kirstin Jarvis get into the honest gap between removing a scar and improving one, and why every operation leaves a mark no matter how skilled the hands that made it. For a cosmetic surgeon, where that line sits and how it heals is the whole job. They clear up one of the most common mix-ups in scar talk: a keloid is not the same as a thick, raised scar, even though most people — and plenty of physicians — use the words interchangeably. The two heal for different reasons and get treated in completely different ways, from steroid and 5-FU injections to, in stubborn cases, low-dose radiation. Dr. Koehler explains why location and skin tension decide whether a scar fades to almost nothing or stays visible, when it's worth re-operating versus treating from the outside with lasers and microneedling, and why time is the most underrated healer of all. He tells two stories — including a woman convinced her beautiful breast reduction scars were ruined — that show how much of the scar conversation is really about expectations. Questions answered by this episode: Can you completely remove a scar, or just move it somewhere hidden? What's the difference between a keloid and a hypertrophic scar? Why do some scars fade beautifully while others stay raised and thick? How do surgeons treat keloid scars?Do drugstore scar creams work as well as medical-grade silicone gel? When is it worth having a scar surgically revised? How long does it take for a surgery scar to fully fade? Why is a tummy tuck scar sometimes thicker than a scar on the face?Do microneedling and laser treatments help flatten and lighten scars? What should I know about scarring before booking a cosmetic surgery consult?Alabama the Beautiful is the cosmetic surgery podcast co-hosted by Dr. Where Did My Butt Go?

You did the work and the pounds are gone, but the skin didn’t disappear. Dr. Vincent Gardner walks through what a full body lift actually does after major weight loss: how a lower body lift combines a tummy tuck with a buttock lift and an auto-augmentation so you don’t end up flat, and how an upper body lift erases the back rolls with an open book tummy tuck that carries the incision all the way around. He also clears up the timing questions people get wrong. You no longer have to wait a full year at a stable weight, it doesn’t matter whether you lost the weight with GLP-1s, bariatric surgery, or diet and exercise, and the single biggest predictor of how well you heal isn’t the surgery at all — it’s whether you’re getting 90 to 100 grams of protein a day. For anyone living inside leftover skin, Dr. Gardner makes the case that this is less about the scars and more about walking out feeling like a different person. Questions answered by this episode: What is a 360 body lift and how is it different from a tummy tuck? What is the difference between a lower body lift and an upper body lift? What is an open book tummy tuck?Do I need a buttock lift and auto-augmentation after weight loss?Should I have my tummy and buttocks done at the same time or separately? How stable does my weight need to be before body lift surgery?Does it matter if I lost weight with GLP-1s, bariatric surgery, or diet and exercise? How much protein do I need to heal after body contouring surgery? How long is recovery after a 360 body lift? Will body lift scars fade, and how do I make them look better?Alabama the Beautiful is the cosmetic surgery podcast co-hosted by Dr. James Koehler and Dr. Vincent Gardner, surgeons with over 2 decades of expertise in cosmetic surgery and their trusty co-host Kirstin, your best friend, confidante, and the snarky yet loveable “swiss army knife” of Eastern Shore Cosmetic Surgery. 2026 Trends: What’s Real and What’s Hype?

Filler migration is all over TikTok, biostimulators get sold as a “natural” alternative, and people are choosing a surgeon by follower count. Dr. James Koehler and Dr. Vincent Gardner react to it all in real time. Kirstin reads off the year’s biggest injectable and body claims and asks for gut reactions. The doctors sort what holds up from what doesn’t: why less filler is winning, why “natural” biostimulators are still just filler with a better publicist, and what a lip flip can and can’t actually do. On the body side, Dr. Gardner explains why microdosing a GLP-1 is probably safer but won’t quiet your cravings, and why a vibrating plate only works if it gets you moving. Dr. Koehler makes the case against quick lunchtime thread lifts and lays out exactly when flying overseas for surgery ends up costing far more than it saves. If a viral video has ever talked you into a treatment, this is the reality check to hear before you book. Questions answered by this episode: Is filler migration real or is the internet overreacting? Why are so many people getting their filler dissolved? Are biostimulators like Sculptra and Radiesse more natural than filler? Is Sculptra reversible if I don’t like how it looks?Does a lip flip actually work or is it just a few units of Botox? Is microdosing Ozempic safe if I’m not overweight?Do at-home body sculpting gadgets and vibrating plates really work? Are lunchtime thread lifts a real alternative to a facelift? What happens if I have a complication after surgery overseas?Should I pick my surgeon based on their follower count?Alabama the Beautiful is the cosmetic surgery podcast co-hosted by Dr. What GLP-1 Weight Loss Does to Your Face (And How to Fix It)

We hear about the weight loss, the body changes, the transformation. What almost nobody mentions is what that same transformation does above the collarbone.  Significant weight loss, whether from medication or lifestyle, can pull volume out of your cheeks, jawline, and neck and leave a face that looks years — sometimes a decade — older than the body it's attached to. Dr. Koehler explains why a fuller face actually reads as younger, especially in people over 50, and why "Ozempic face" isn't really about the medication at all. The culprit is the scale of the weight loss. Lose five pounds and your face probably looks fine. Lose 40 or 50 and you may not recognize what's looking back at you. When it comes to solutions, the call is never one-size-fits-all. Dr. Koehler separates two distinct problems — volume loss and skin laxity — and explains why fixing one without addressing the other rarely gives a satisfying result. He walks through when filler makes sense, how facial fat grafting actually works (including the role of platelet rich plasma), and when a facelift is the only thing that will do the job — even in someone in their 30s or early 40s. There's also the timing question. Should you wait until you hit your goal weight before doing anything? What if you plan to stay on GLP-1s indefinitely? And what do you do if you're watching your face change month over month right now? Dr. Koehler has a clear framework for all of it. Questions answered by this episode: What is Ozempic face and is it actually caused by GLP-1 medications? Why does losing weight make your face look older? How much weight loss does it take before you see it in your face?Does skin tighten on its own after major weight loss? What is the difference between filler, fat grafting, and a facelift for facial volume loss? How does facial fat grafting work and where does the fat come from? Can someone in their 30s or early 40s be a facelift candidate?Should you wait until you reach your goal weight before getting a facelift? If you plan to stay on GLP-1s long-term, does that change what a surgeon recommendsWhen would a surgeon recommend cheek implants instead of fat grafting?Alabama the Beautiful is the cosmetic surgery podcast co-hosted by Dr.
