  • Rachel’s GLP 1 Journey After Cancer, Weight Regain and Starting Over

    23 Sept

Rachel’s GLP 1 Journey After Cancer, Weight Regain and Starting Over

Rachel has lost 100 pounds more than once. She has also regained it, gone through cancer treatment, dealt with steroids, stress, major life changes, and reached a point where the strategies that had worked for her in the past were no longer giving her the same results. In this conversation, Rachel shares what led her to start exploring GLP 1 medications, what happened when insurance would not cover the prescription her doctor wrote, and why her first experience with a compounded option left her feeling extremely sick and unable to eat. We also talk about what changed when she found an approach that felt more manageable, why she views GLP 1 medication as another tool in the toolbox, and how she responds to people who call medication assisted weight loss “cheating” or “the easy way out.” Rachel is very open about emotional eating, food addiction, weight regain, social media judgment, and the reality that knowing what worked before does not always mean it will work the same way now. One of the biggest themes in this conversation is adaptability. Life changes. Bodies change. Circumstances change. The plan may need to change too. Rachel is not promising that this is the final answer forever. She is sharing what is helping her right now, what she has learned along the way, and why she continues to focus on movement, nutrition, and using the tools available to her while figuring things out one step at a time. • What If Food Noise Comes Back After a GLP-1?

    15 Sept

What If Food Noise Comes Back After a GLP-1?

What happens when a GLP-1 gives you relief from Food Noise, but part of you is already worried about what happens if that relief changes? I think that fear is incredibly understandable. If you spent years or decades thinking about food, dieting, losing weight, regaining weight, and trying to control your eating, having some of that mental noise become quieter can feel like a huge relief. It also creates a new question. What happens if it comes back? In this episode, I talk about why the fear may not only be about Food Noise itself. It may be about what you think the return of Food Noise means. Will I regain the weight? Will I lose control? Will I fall back into old habits? Will I become the version of myself I was trying to get away from? I also get into something I think some people may quietly wrestle with after losing weight on a GLP-1. Did I take the easy way out? I do not believe using a GLP-1 means you took the easy way out. You used a tool that helped you. But if part of you believes otherwise, I think it is worth knowing. Because if you secretly believe the medication did all the work, the thought of stopping it or having its effects change can understandably create a lot of fear. I also talk about why I do not think every period of relief needs to immediately become another self-improvement project. If Food Noise has gotten quieter, you are allowed to enjoy that. You do not necessarily have to immediately build the perfect eating plan, perfect exercise routine, perfect maintenance strategy, and perfect mindset. When you are ready, you can start figuring out what works for you. And if Food Noise does come back, that does not mean you are back at square one. You can describe what you are experiencing. You can notice what it means to you. You can decide what you want to do next. Your past does not have to become your future simply because an old thought returns. New to Your Level Fitness? • Staying on a GLP-1 After Losing Weight: How Do You Decide What Comes Next?

    2 Sept

Staying on a GLP-1 After Losing Weight: How Do You Decide What Comes Next?

In this episode of Life During and After GLP-1, I’m joined by Jennifer to talk about her personal experience with GLP-1 medication, from the decision to start through weight loss, side effects, rebuilding muscle, and eventually reaching the question of what comes next. Jennifer shares how her fitness routine changed during COVID, how weight regain eventually led her to begin researching weight loss medications, and what happened when the cost and availability of name-brand medication created another obstacle. We talk about her experience using compounded medication, how quickly she noticed changes in how much she could eat, and some of the tradeoffs she experienced along the way. While she didn’t deal with many of the commonly discussed gastrointestinal side effects, she did notice changes in the foods she could comfortably eat, along with hair loss and significant muscle loss. That loss of strength eventually became a turning point. Jennifer shares what happened when she returned to strength-focused group workouts, how much stronger she feels now, and why rebuilding muscle became an important part of her life after losing more than 50 pounds. We also get into something that may become one of the biggest questions for people as GLP-1 use continues to evolve. What happens once you have lost the weight and feel good where you are? Jennifer is now navigating the balance between staying on a maintenance dose and wondering whether she eventually wants to stop taking the medication. We talk about the fear of weight regain, her continued relationship with the scale, rebuilding strength, food noise, and the mental side of deciding what maintenance should actually look like. Most importantly, this is Jennifer’s experience. GLP-1 journeys can look very different from person to person, and her advice for anyone considering medication is simple. Do your research, understand where your medication is coming from, and work with qualified healthcare professionals who can appropriately oversee your care. #411 ‒ AMA #90: Strong convictions, loosely held: how Peter's views on fasting, keto, metformin, rapamycin, whole-body MRI, and exercise have changed

View the Show Notes Page for This Episode Become a Member to Receive Exclusive Content Sign Up to Receive Peter's Weekly Newsletter In this "Ask Me Anything" (AMA) episode, Peter looks back on 400 episodes of The Drive with another installment of "strong convictions, loosely held," revisiting some of the positions he has taken over more than eight years of the podcast. He explains why changing one's mind in response to new evidence, conversations with experts, and clinical experience is essential for practicing medicine well. Peter begins with nutrition, describing how his views on ketogenic diets, extended fasting, and the framework of dietary restriction, time restriction, and calorie restriction have evolved. He explains why he no longer takes metformin for longevity and discusses his growing uncertainty around rapamycin despite compelling animal data. Peter also revisits his views on cancer screening, including whole-body MRI and liquid biopsies for early detection. On exercise, Peter explains how his approach to cardiorespiratory training and the 80/20 Zone 2 framework has changed, particularly for people with limited time to train. He also shares why he has moved away from heavy barbell squats and deadlifts and how he now thinks about balancing benefit and injury risk. Throughout the episode, Peter emphasizes the importance of focusing on durable principles, strong foundations, and sustainable practices rather than chasing optimizations that provide marginal gains. If you're not a subscriber and are listening on a podcast player, you'll only be able to hear a preview of the AMA. If you're a subscriber, you can now listen to this full episode on your private RSS feed or our website at the AMA #90 show notes page. If you are not a subscriber, you can learn more about the subscriber benefits here. We discuss: Strong convictions, loosely held: changing opinions as evidence, expert conversations, and clinical experience evolve [2:00]; Nutrition: Peter's evolving view of ketogenic diets, energy balance, and dietary dogmatism [5:30]; Extended fasting, autophagy, lean-mass loss, and who may benefit from prolonged fasting [12:00]; The dietary restriction, time restriction, and calorie restriction framework—and why GLP-1 agonists are tipping the balance toward calorie restriction [20:15]; Why Peter no longer views metformin as geroprotective for metabolically healthy people [24:45]; Rapamycin for longevity: compelling animal data, unresolved human dosing, and Peter's growing uncertainty [29:15]; The importance of early cancer detection, declining whole-body magnetic resonance imaging (MRI) quality, and more targeted alternatives [41:30]; Liquid biopsies for cancer screening: the NHS-Galleri trial, why early-stage rather than late-stage detection is what actually matters, and why the technology remains "fuzzy" [53:30]; Evolving exercise goals, the limitations of the 80/20 Zone 2 framework, and programming cardio at lower training volumes [1:00:00]; Exercise prescriptions for limited time, building a training habit, and becoming less dogmatic about forcing workouts [1:05:00]; Moving away from heavy squats and deadlifts, alternative exercises, and deliberate risk management [1:13:00]; Prioritizing principles, foundations, and sustainable practices over marginal optimization [1:21:30]; and More. 4 food groups to lower inflammation, support your immune system and help you age well | Dr Jeffrey Bland

Every time you sit down to eat, you’re giving your body information. Compounds in your food act as signals that influence your gut microbes, immune response and even the expression of your genes. That’s the idea we explore today with biochemist Dr Jeffrey Bland, widely known as the father of functional medicine. He co-founded the Institute for Functional Medicine and wrote The Disease Delusion, one of my all-time favourite books! We bring the research back to your kitchen with four food priorities for gut health, immune resilience and healthy ageing: polyphenols, fibre, healthy fats and fermented foods. Plus, why we all need to eat more bitter foods! In this episode: How food can influence gene expressionThe link between immune health, metabolism and ageingPolyphenol-rich foods and why variety mattersFibre and fermented foods for gut healthOmega-3s, olive oil and inflammationFish versus fish oil supplementsBitter foods and how they may help regulate appetite If you want to put food as medicine on your plate, try The Doctor’s Kitchen app for free.
