Does TRT Shrink Your Testicles? What to Know

Send us Fan Mail - Bigger testicles don’t mean more testosterone. Most of their volume is actually devoted to making sperm. Bigger testicles don’t mean more testosterone. So what does testicle size actually tell you about your hormones, your fertility, and your performance in bed? On The SexMD Podcast, Dr. Debra Durst and RevitalyzeMD provider Breanna Case walk through 10 facts about testicle size that most men never hear. They cover what the testicles actually do, how testosterone therapy affects them, how to keep them working while on TRT, and what women really think about size. If this conversation resonates, hit like and subscribe — it helps us bring more honest, physician-led conversations on hormones, intimacy, and confidence to more people. What You'll Learn in This Episode - Most of the testicle’s volume goes to sperm production, so size reflects sperm capacity more than testosterone. Normal size ranges from about 15 to 25 mL each, and some asymmetry is common. - Testosterone replacement therapy can shrink the testicles. hCG, clomiphene, and enclomiphene can help keep them producing. - Function can recover after stopping. In one study of more than 1,500 men, sperm production recovered in 90% within 12 months. - Bigger testicles don’t mean better erections or better sex. Any sudden change in size, pain, or a firm lump should be checked by a doctor. Have a question about testosterone or testicle health? Leave it in the comments. Dr. Durst is always open to a deeper dive. JoyLux, Red Light device: Halle Berry says, “Juicy like a peach”

Send us Fan Mail Halle Berry is talking about menopause and sex, and she’s partnered with a red light device women use at home. So what’s the science behind it? Halle Berry is bringing intimate wellness into the menopause conversation through her partnership with Joylux. So what does red light therapy actually do for arousal, comfort, and vaginal health? Dr. Debra Durst explains the science behind the Joylux vFit, an at-home intravaginal device that combines red light, gentle heat, and vibration. She covers how it compares with the in-office vaginal laser treatments used at RevitalyzeMD, who may benefit, and how the consumer and provider-only versions differ. If this conversation resonates, hit like and subscribe — it helps us bring more honest, physician-led conversations on hormones, intimacy, and confidence to more people. What You'll Learn in This Episode - Female sexual dysfunction ranges from low libido to pain with sex. Surveys show it affects about 41% of premenopausal, 52% of perimenopausal, and 64% of postmenopausal women. - Red light therapy works through photobiomodulation, which can support cellular energy, blood flow, collagen, and nerve health. - In-office vaginal lasers for intimate wellness aren’t the same as ablative CO2 lasers. They gently heat the tissue to trigger repair without significant downtime. - Joylux is used every other day at home to support in-office results, or as a starting point for women not yet ready to see a doctor. The vFit Plus, available through medical providers, delivers about 30% more energy and longer sessions. Have you used Joylux? Share your experience in the comments. Dr. Durst is starting the device herself and will share more in upcoming videos, so subscribe to follow along. #RedLightTherapy #Joylux #Menopause #SexualWellness #VaginalHealth Start Your Transformation — Grab Your Free Guide 📘  Perimenopause vs. Menopause Guide Finally understand your symptoms and get a roadmap to feeling balanced, energized, and confident again. 👉 Download Free: https://offers.revitalyzemd.com/perimenopause-guide These guides give you the same foundational education we provide to patients in the office—so you can start improving your health before you ever book an appointment. Your Hormone Levels Are Normal… So Why Do You Feel Terrible?

Send us Fan Mail “Just because it’s normal doesn’t mean it’s good. I’d run your panel, tell you your labs are normal, and then tell you to find a specialist who can actually help you.”  This comes from a former family medicine provider admitting the system’s limits, and it speaks directly to every viewer who’s been told “your labs are fine” while still feeling off. Your hormone labs came back “normal,” but you still don’t feel like yourself. Here’s why normal isn’t always the same as optimal, and why insurance often influences your treatment options before you even have a say. In this episode of The SexMD Podcast, Dr. Debra Durst sits down with Bre Case, the newest provider at RevitalyzeMD, to talk about why she left family medicine and chose a different approach to patient care. They discuss how insurance shapes hormone therapy decisions for men and women, why perimenopause is so often overlooked, and what changes when patients and providers work together to create a personalized treatment plan. If this conversation resonates, hit like and subscribe — it helps us bring more honest, physician-led conversations on hormones, intimacy, and confidence to more people. What You'll Learn in This Episode - Normal lab ranges are broad, not optimal. Many men and women stay symptomatic within them, and insurance often won’t cover testosterone or perimenopause treatment until labs fall outside that range. - Asking about erectile dysfunction is a powerful health screener, because the smallest blood vessels often show early signs of high blood pressure, cholesterol, and diabetes. - Research can take about 17 years to reach routine care. With menopause, that delay has already cost a generation of women effective treatment. - Sexual health is part of overall wellness, and every patient deserves to be asked about it. If you're curious about hormone optimization, why perimenopause is so often overlooked, or how insurance impacts your treatment options, this episode is for you. Penis Pumps: Why a Sex Store Pump Isn’t Enough

Send us Fan Mail You can buy a penis pump at your local sex store, but that’s not the same thing as a medical-grade vacuum erection device. Not every penis pump is a medical device. Dr. Debra Durst explains the difference between a sex-store penis pump and a medical-grade vacuum erection device, and why it matters for erectile dysfunction and long-term erection health. In this video, Dr. Durst covers who vacuum erection devices are for. That includes men noticing changes in their erections and men rehabilitating after prostate surgery. She explains how she pairs pumping with in-office regenerative treatments like acoustic shockwave and P-Shots. She also walks through the two cylinders of the Kaplan Pump “Whopper” and how to use each one safely. If this conversation resonates, hit like and subscribe — it helps us bring more honest, physician-led conversations on hormones, intimacy, and confidence to more people. What You'll Learn in This Episode - Medical-grade vacuum erection devices, some of which are FDA 510(k) cleared, include a pressure gauge and a release valve that most novelty pumps lack. - Pump gradually to an erection. It should never hurt, cause bruising, or push the gauge into the red. - During regenerative treatment, Dr. Durst recommends about 10 minutes of pumping twice a day to bring healthy blood flow into the tissue. - There’s good data supporting girth gains with pumping, but length gains are not well documented. #ErectileDysfunction #PenisPump #VacuumErectionDevice #MensHealth #SexualWellness Interested in this product? Low Libido After Menopause: Why No Doctor Ever Asks

Send us Fan Mail Seven women. One doctor. Not one of them had ever proactively asked how her sex life was — and that silence is exactly what's driving today's conversation about low libido, hormones, and menopause. In this roundtable, Dr. Debra Durst sits down with a panel of women to talk candidly about what actually happens to desire, arousal, and intimacy during perimenopause and menopause — the conversations most doctors' offices never make room for. The panel gets into vaginal dryness, painful sex, the guilt that comes with a vanishing sex drive, and why so many women suffer through it in silence simply because no one ever asked. A few key takeaways from this conversation: - Roughly 80% of women don't experience spontaneous desire — arousal has to come first, and that's normal physiology, not a problem to fix - Vaginal dryness and painful sex are common but not something you have to live with, even on hormone therapy - Low libido is one of the most common — and most under-discussed — symptoms of hormonal changes, and it can improve significantly with proper optimization - Opening the conversation with your partner (and your doctor) changes everything, including the relationship itself - If you've ever felt alone in this, you're not. Subscribe for more from this roundtable series, and check out the full conversation on The SexMD Podcast. Hormone Imbalance: 5 Signs It’s More Than Fatigue

Send us Fan Mail Feeling tired doesn’t automatically mean low testosterone. Gaining weight doesn’t automatically mean menopause. Feeling tired, gaining weight, or losing your sex drive doesn’t automatically mean you have a hormone imbalance. When several of these symptoms show up together, though, it may be time for a hormone evaluation. In this video, Dr. Debra Durst walks through the five signs her patients describe most often: changes in energy, body composition, sexual function, sleep and mood, and menstrual cycles. She explains how estrogen, progesterone, testosterone, and thyroid hormones can affect both men and women. She also explains why the same symptoms can come from sleep apnea, anemia, insulin resistance, medications, or stress. If this conversation resonates, hit like and subscribe — it helps us bring more honest, physician-led conversations on hormones, intimacy, and confidence to more people. What You'll Learn in This Episode -Fatigue is non-specific. Thyroid disease, anemia, and sleep apnea can look exactly like a hormone problem, so get evaluated instead of guessing. -Gaining fat around the middle while eating and exercising the same way can be linked to menopause in women and to low testosterone in men. -For men, changes in erections can be an early signal about vascular health. - Irregular cycles, hot flashes, night sweats, and vaginal dryness are classic signs of perimenopause. Is Mouthwash Bad for Sex?

Send us Fan Mail Would you take an antibiotic every day for the rest of your life to kill the bacteria in your gut? Of course not. But that's exactly what most of us do to our mouths. Your mouthwash may be quietly undermining your blood flow. On this episode of The SexMD Podcast, we explore how the oral microbiome drives nitric oxide production, and what that means for blood pressure, erections, and sexual wellness.  Dr. Debra Durst sits down with Dr. Haraldo Magaranos, a biologic dentist, periodontist, oral surgery specialist, and naturopathic doctor. They cover: -How the mouth and gut microbiomes communicate -Why antiseptic mouthwash and conventional toothpaste may do more harm than good -How mouth breathing and acid-reducing medications disrupt nitric oxide -The hidden toxic burden many patients carry, including from mercury fillings Key takeaways: -Nitric oxide shifts with age. the body's endothelial nitric oxide production drops to less than 50%. That makes the oral bacteria pathway, which converts dietary nitrates, increasingly important for blood flow. -Killing mouth bacteria has measurable effects. Common antiseptics like chlorhexidine can raise blood pressure 3–4 points by wiping out the microbes that help produce nitric oxide. -Your mouth seeds your gut. You swallow roughly 140 billion microbes a day in saliva, directly influencing the gut microbiome that modulates about 70% of immune responses. -Clean without sterilizing. Mechanical cleaning like brushing and flossing, a microbiome-friendly toothpaste, nasal breathing, and nitrate-rich foods support the system instead of fighting it. Enlarged Prostate: Stop Getting Up 3 Times a Night

Send us Fan Mail Getting up two or three times a night to urinate? A weaker stream, or waiting for things to get started? An enlarged prostate (BPH) may be the cause, and there are things you can do about it. In this video, Dr. Debra Durst explains why the prostate grows, how DHT and other hormonal changes drive that growth, and why it squeezes the urethra like a pinched garden hose. She covers the warning signs of benign prostatic hyperplasia, simple lifestyle changes, prescription options, and where supplements like pygeum fit in. Key takeaways: -BPH is driven largely by DHT, a hormone that strongly signals prostate cells to grow.  -Finasteride and dutasteride work by blocking how DHT is made. -Low-dose tadalafil (Cialis 5 mg) is FDA-approved for BPH urinary symptoms and can also help erectile function. -Small changes can ease mild symptoms: cut back on fluids, caffeine, and alcohol in the evening, try double voiding, and review cold and allergy medications. -Any change in your urinary pattern is worth getting checked. BPH isn't the only possible cause. -Don't accept a weak stream or constant bathroom trips as normal.
