    Insomnia with Dr. Muhammad Usama

Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com) Do you actually know the difference between a rough night's sleep and a real sleep disorder? Most people use the word "insomnia" for both, but the line between them is a lot more specific than it sounds — and knowing where that line sits can change how someone approaches their own sleep struggles entirely. That's exactly what this episode digs into, with Dr. Muhammad Usama returning to the show after a prior conversation on Zepbound and sleep apnea. He's triple board-certified in sleep and obesity medicine, practices at MultiCare Health System in the greater Seattle area, and serves as an adjunct clinical professor of medicine at A.T. Still University School of Osteopathic Medicine. His clinical and research work centers on the intersection of obstructive sleep apnea, obesity, and cardiometabolic health, with a particular focus on GLP-1 therapies like tirzepatide. This time, the conversation turns to insomnia itself: what actually qualifies as chronic insomnia, why it happens, and what treatment options genuinely move the needle versus which ones just create the illusion of help. Topics discussed: When Does Insomnia Stop Being Normal? The Switch That Turns Short-Term Insomnia Into Chronic InsomniaIs Insomnia Genetic? The Nature vs. Nurture AnswerChronic Insomnia: Why CBT-i, Not Sleep Hygiene, Is the First-Line TreatmentHow to Actually Access CBT-i (No Therapist Couch Required)Do Sleep Medications Actually Work? The Research Says Not MuchAcute Insomnia: How Early Intervention Prevents It From Becoming ChronicDoes Melatonin Actually Help With Sleep? What It's Really ForDo Sleep Supplements Like Magnesium Actually Work?Alcohol and Marijuana for Sleep: Do They Actually Help—or Hurt?Chronic Insomnia: When to Get Evaluated and What Actually WorksInsomnia is far more common, and far more treatable, than most people realize. Between understanding the difference between acute and chronic, recognizing the habits that quietly turn one into the other, and knowing which treatments actually hold up under research, this episode covers a lot of ground that can change how someone approaches their own sleep for good. If sleep has been a struggle for months, it's worth getting evaluated rather than just living with it — this episode is a great place to start. Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com) Do you know what happens in your body when you eat a meal? Or did you, like most of us, make it to adulthood without anyone explaining the basics? If that question made you pause, you're not alone. Most "healthy eating" advice arrives in fragments — a rule from one diet trend, a warning from another — without ever adding up to a full picture of how food actually works in the body. Protein, carbs, fat, vitamins, minerals, phytonutrients: these aren't competing categories to master one at a time. They're a system. Once you understand how the pieces fit together, meal planning stops feeling like guesswork and starts feeling like common sense. This episode covers exactly that — what each macronutrient is doing for you, why micronutrients matter even in small amounts, and how to build a plate that supports your body without obsessing over any single food or number. No fad diets, no miracle foods. Just the fundamentals, explained clearly. Topics discussed: Macronutrients: The Building Blocks of Your DietProtein: Why Your Body Needs ItCarbohydrates: Choosing Better Sources of EnergyFat: Why It's Essential for Good HealthCaloric Density: Why Some Foods Contain More Calories Than OthersMicronutrients: The Vitamins and Minerals Your Body NeedsWhole Foods: Why Variety Matters More Than Individual NutrientsBuilding a Balanced Plate for Better NutritionReducing Added Sugar Without Feeling RestrictedEating More Protein to Stay Fuller LongerFocus on Adding Healthy Foods Instead of Restricting ThemCreating Healthy Eating Habits That LastYour plate is the one thing you control every single day — three times a day, for the rest of your life. Every meal is a chance to give your body what it actually needs instead of guessing, restricting, or chasing whatever the latest trend says to avoid. JAMA Secret Shopper and What Obesity Care Should Actually Look Like

Claim your complimentary gift of my exclusive mini weight care guide today! Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com) Have You Ever Wondered If Your Telehealth Weight Loss Clinic Is Actually Giving You Real Medical Care, or Just a Prescription? This episode dives into a JAMA study that's been generating serious buzz throughout the obesity medicine community, and once you hear what it uncovered, you'll understand exactly why. Researchers conducted a "secret shopper" investigation, sending someone posing as a patient to 49 different telehealth clinics offering obesity management medications, simply to see what kind of care they'd actually receive. The findings shed light on where the gaps really are in this rapidly expanding area of healthcare, and they back up what a lot of obesity medicine providers have suspected for quite some time. We're unpacking what comprehensive obesity care is truly supposed to look like, and how you can figure out whether the care you're getting, or the care you're offering, actually holds up. And there are two calls to action woven throughout: if you're someone taking these medications or considering starting them, we'll walk through how to find a provider who's doing this the right way. And if you're a clinician providing this kind of care already, consider this your reminder to put yourself out there more, because there are patients who genuinely need access to someone doing it well. Topics Discussed: The JAMA Study Raising Questions About Obesity TelehealthThe Missing Piece in Many Telehealth Weight Loss ProgramsConvenience Shouldn't Replace Comprehensive Medical CareThe Importance of Clinical Oversight in Telehealth PrescribingWhat Comprehensive Obesity Care Should Look LikeWhere to Start If You're Considering Weight-Loss MedicationHow to Find High-Quality Obesity CareThis episode covers a lot of ground, from the specific numbers behind the study to what a truly thorough evaluation should include, and what red flags to watch for if you're navigating this kind of care yourself. Whether you're a patient trying to figure out if your provider is doing right by you, or a clinician who wants to understand where the industry is falling short, this conversation breaks it all down in a way that's genuinely worth your time. Press play to catch the full episode and get every detail we covered. Want to see the data for yourself? Cut Back Screen Time to Improve your Health and Weight

Claim your complimentary gift of my exclusive mini weight care guide today! Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com) What if the biggest thing standing between you and your health goals wasn't food, or exercise, but the screen you're holding right now? Today we're talking about screen time — how too much of it affects your health, your weight, and what you can actually do about it. I've got five problems screens create, and five solutions to go with them, so stick with me. Screen time isn't something I bring up with patients as often as I probably should, but it shows up in two specific places in my practice. The first is during a comprehensive annual wellness visit, where I think about screens the same way I think about substances like alcohol, tobacco, or marijuana — something we tend to overuse, often to numb out or fill a void, and something we can usually cut back on to genuinely improve our health. The second is in weight maintenance. Once a patient reaches their goal weight and we shift into maintenance mode, I walk through a handout built around the National Weight Control Registry — a long-running study tracking people who lost 30 or more pounds and kept it off for at least a year. One of the most telling data points from that registry: 62% of people who successfully maintained their weight loss were watching less than 10 hours of TV a week. That's still over an hour a day, so there's plenty of room for your shows — but it tells us something real about the role screens play in long-term success. Today we're getting into exactly why. Topics discussed: The Sneaky Way Screen Time Is Draining Your Daily MovementWhy Screen Time and Snacking Go Together More Than You RealizeThe Hidden Link Between Blue Light, Sleep, and WeightThe Stress Connection Nobody Talks About With Screen TimeThe Real Cost of Letting Screens Eat Your Free TimeOne Easy Shift That Actually Cuts Down Screen TimeTry the 30/30 Rule to Break Up Long Periods of SittingKeep Screens Out of MealtimesSet a Screen Curfew Before BedPair Screen Time With MovementThere's a lot packed into this one, and every piece builds on the last. We'll get into why your favorite show might be quietly killing your daily movement, how it sneaks into your snacking and sleep without you noticing, and how something as small as the 30/30 rule or a bedtime curfew can start turning that around. Press play and find out where your screen time might be costing you more than you think. Five New Shifts in How Exercise Affects Health

Claim your complimentary gift of my exclusive mini weight care guide today! Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com) Ever feel like exercise gets treated as this magic weight-loss tool, when really that's not even its main superpower? Here's the truth: most people don't lose significant weight from exercise alone. Your body is smart (annoyingly so) — it adapts, burns fewer calories than you'd expect, and can even nudge you to move less throughout the day or eat a little more to compensate. So if you've ever felt discouraged because the scale didn't move despite all your effort at the gym, you're not doing anything wrong. That's just physiology doing its thing. But here's the part that matters even more: exercise isn't about the scale, and it never should be. It's genuinely one of the most powerful things you can do for your overall health, no matter your weight. It also plays a major role in maintaining weight loss once you've achieved it, arguably a bigger win than losing the weight itself. Exercise was never meant to be a punishment for how you look, it's a tool for feeling stronger, sleeping better, and building a body that can support you for the long run. This week's episode dives into five new conversations we should all be having about exercise, from the real sweet spot for strength training to the surprising ways your muscles work behind the scenes: Topics discussed: The Sweet Spot for Strength Training, According to ResearchThe Case for Mixing Up Your WorkoutsMuscle Mass vs. Muscle Function: What Actually Matters on GLP-1sThe Minimum Effective Dose: How Little Exercise Actually Counts? The Endocrine Organ You Didn't Know You HadPress play to catch the full breakdown, plus practical ways to nudge your own routine forward, no matter where you're starting from. Genetics of Obesity with Dr. Jesse Richards

Claim your complimentary gift of my exclusive mini weight care guide today! Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com) What If Your Weight Struggle Was Never About Willpower? What if the reason some people struggle with their weight their entire lives has nothing to do with what they're eating or how hard they're trying — and everything to do with their DNA? It's a question that doesn't get asked nearly enough, and today, we're finally diving into it. For so many people living with severe obesity, the missing piece of the puzzle isn't a better diet plan or more motivation. It might be written right into their genetic code — and that changes everything about how it should be understood and treated. Today's guest is Dr. Jesse Richards — board-certified in internal medicine and obesity medicine, Director of Obesity Medicine at the University of Oklahoma School of Community Medicine, and the person his colleagues call when they have questions about the genetics of obesity. His research covers genetic obesity, GLP-1s, and novel treatments for obesity and substance use disorders. Safe to say, he knows this space better than almost anyone.
